SAFETY Safety in the operation and maintenance of power system facilities is most important to prevent accidents or incidents which will result in injury or fatalities to employees. In view of this, Operating and Maintenance staff employ all available means to avert such accidents and incidents. One such method is the employment of standard ‘Definitions of Special Terms” which are used by Operating and Maintenance personnel before, during and after work on any apparatus under the control of the Operating staff. Protection Guarantee of various forms are issued to maintenance personnel by Operating staff for work on apparatus and communication (both by telephone and direct conversation) must be approached with special terms which is understood by all parties.

ACCIDENTS ACCIDENTS may be defined as an unplanned event which results in:  Harm to People  Damage to Property  Loss of Process SAFETY

ACCIDENTS NEAR MISS also known as Near Accident may be defined as an unplanned event which could have caused harm under slightly different conditions SAFETY

REASON WHY MINOR ACCIDENTS AND NEAR MISS ARE NOT REPORTED  Fear of Disciplinary action  Lack of Management commitment  No training provided on investigation  Too much paper work SAFETY

REASON WHY MINOR ACCIDENTS AND NEAR MISS ARE NOT REPORTED  No incentive to report minor accidents and near misses  Disincentive for reporting minor accidents and near misses  Reporting near misses hurts the department’s safety performance and reduces safety related bonuses. SAFETY
