Near Miss Reporting Program

Definition An event that could have caused harm or damage, But did not. Objective/ Purpose To encourage the reporting of near misses by hourly and supervisors

Procedure for Reporting Employees are encouraged to report near misses in the following manner A: when observing or experiencing a near miss, fill out the ballet then hand it into there supervisor B: The supervisor then will determine if it’s a near miss or incident before dropping off into the ballot box.

Procedure When observing or experiencing a near miss, fill out the “Near Miss Report Form”. Place form in ballet box, located in the wicket. Supervisors, will check the near misses at the end of every shift. Record the near misses on the white boars for review by the cross shift and for their crew the next day.

All near misses will be entered into Minesafe and forwarded to the safety department. Once the near misses are received, the safety personnel will rate the potential severity of the near miss to determine if the near miss requires further investigation. (using the loss potential severity chart).

Recognition Process At the end of every week the safety department will draw one near miss from all the reports submitted for the week and the employee will receive a voucher. All near misses will then be entered into a quarterly draw and yearly draw.

-While bolting on a Maclean deck the operator noticed the deck side shifting on its own narrowly pinching his partners hand on the rail. He shut down and called maintenance dept to repair the problem. Action Taken - Action Taken: The machine was tagged out until repaired.

On March 19, 2008. EE was exiting dry when he almost slipped on stairs and could have fallen. Actions Taken: Dry personnel to start ½ earlier to remove all snow from all entrances into offices and dry’s, also grates were added to the walk ways

On July 16 2009 an employee noticed the fire ext. on his scoop didn’t seem to sit properly. Upon further inspection the bracket had opened up enabling the fire ext. to possibly fall out. On Jan 2 2011 an employee noticed a haulage truck’s box rubbing against the wall pealing back the screen. The employee cut out the damaged screen with bolt cutters making the area safe for others.
