On your mark. Get set. (You should be under the Services tab clicking the Family Application tab and highlighting the name of the child completing the enrollment.) GO!!

Make sure all the blanks are complete in section 1. (General Information)

Make sure the address and phone are correct. 1. If the address is not correct, click on family information (in blue) and update the information and complete a change of status (COS). Add the COS under the family services tab and place it in the CHILDPLUS FOLDER for verification by monitoring team. 2. If the phone number is not correct, click add Phone number (in blue) and update the information and complete a change of status (COS). 1.Make sure Family Information is highlighted NOT child (updated August 2010) 2.Click Add Contact ( in blue) and add information. 1. 2.

Add all the information and save. Make sure ask the parent/guardian if the person is an emergency contact and/or a release person. Save the information.

1. Make sure the child’s name is highlighted in the Health Tab. 2. Click Doctor/Medical Home to select the doctor’s information.

1. Highlight the Doctor/Medical Home name. 2. Click Select. The information will be added. If the doctor is not listed, email or call Mary Ramos with the name and information for the doctor and she will add the information to CHILDPLUS.

1. Make sure the child’s name is highlighted in the Health Tab. 2. Click Dentist/Dental Home to select the dentist information.

1. Highlight the Dentist/Dental Home name. 2. Click Select. The information will be added. If the doctor is not listed, email Mary Ramos with the name and information for the doctor and she will add the information to CHILDPLUS.

1.Make sure the child’s name is checked in the Health Tab. 2.Double check to see that the Health Coverage section is filled out.

Keep Going!!!!!! Head to the Family Services Tab.

You’re almost there!!! You can do it!!! Head for the PIR Addendum (under the Family Services Tab –Family Services Information tab)!

Ask the parent completing the enrollment if they have any needs for each of the requirements listed. Mark yes or no, according to the parent’s answer.

1.Print the enrollment application for parent’s signature (and yours). 2.Make sure you attach the enrollment contact note. 3.Put the enrollment in the CHILPLUS FOLDER for verification. 4.Once monitors verify information they email Mary to enroll child, she then calls you to know which classroom

As you cool down, there are additional items you need to add to CHILDPLUS, before you relax.
