4 Please buy and read “The Choice is Yours” by John Maxwell. 7 th Grade “Dare to Dream” by John Maxwell. 8 th Grade It would be helpful for you to go through this book with them. If finances are a concern, please see Coach Harden after the meeting. SUMMER READING

5 Student Expectations Must maintain a 70% or above in academic classes Athletes will be role models on campus Repeated violations of the Colt Commitment Contract will result in the athlete being removed from the athletic program

6 Athletic Forms Physicals and Medical History- must be turned in before football equipment can be issued or participate in any sport or practice Athletic Forms- INFO SNAP

Paperwork: Medical History and Physical- hard copy ***CMS MUST HAVE TO HAND OUT EQUIPMENT*** Athletic District Online Forms- INFO SNAP Acknowledgement of Rules Steroid Concussion Acknowledgement Apparel- Must Have Everyday Black shorts Grey shirt Cleats Tennis shoes Cold Weather -School Colors Covers all skin Sweatshirt Sweatpants Football Parents provide: Cleats (School colors only), white practice pants (with pads),Game Pants (black), and mouthpiece if students desire their own design (we have plain). School Provides: Helmet, Shoulder pads, and jerseys, mouthpiece if needed
