Some points on lagging indicators Hydrocarbon releases –rising trend in >25kg releases –compression systems & small-bore piping significant contributors –human factors a significant cause No fatalities, major injury rate 10% up on last year Over-3-day injury rate 10% down on last year Dangerous occurrences 5% up, only 3.6% better than 12 years ago 37% of all injuries from maintenance/construction 19% of all injuries from deck operations
