8 The CEO/Accounting Officer and EE Manager will be sent an email with a reminder letter. The reminder letter contains a link to activate your account. The employer will be prompted to change their password. Passwords are only valid for one reporting period. 8

13 DG NOTIFICATION (EEA14) Employers who are unable to submit their EE reports must apply using EEA14 form. Section 21(4A) Act 47 of 2013 and EE Regs 2014. EEA14 applications must be submitted before the last working day of August. The reasons provided by the applicant must be consistent with the EEA14 form. The forms must be accompanied by a motivation, supporting documents and signed by the CEO/Accounting officer. This form cannot be used by employers to request exemption to reporting or condonation for failure to report. 13

14 THINGS TO REMEMBER All designated employers regardless of the size must report annually during the period 1 September to 15 January 2016. Verify EEA2 & EEA4 forms to ensure correctness of information. The CEO/Accounting officer must approve the EEA2 form and the EEA4 form before submission. Email addresses appearing on Sec A of the EEA4 must be of the CEO and EE manager. The employer must retain a copy of the approved and signed EEA2 and EEA4 for a period of 5 years. The employer must inform the Department in writing regarding changes e.g company name, contact details, mergers/acquisition and request for deregistration. 14
