20 Air Traffic Services – DP4 Mr. David Moores, ICAO Regional Officer ATM attended the meeting to provide technical input from an ICAO perspective Meeting identified the need for COSCAP to establish a generic Safety Management System for ATM which could then be adapted by Member States COSCAP could assist further with the implementation of the safety management programme ICAO has indicated that they should be able to support this initiative to some extent COSCAP North Asia Cooperative Development of Operational Safety & Continuing Airworthiness Programme

26 NARAST Report – DP5 Statement of Work:(TAWS) Controlled flight into terrain (CFIT) - accidents, where a properly functioning aircraft under the control of a fully qualified and certificated crew is flown into terrain with no apparent awareness on the part of crew, could be substantially reduced or eliminated with the installation of TAWS equipment. 27 NARAST Report – DP5 TAWs (Rec. 31.2 ) – COSCAP-NA Advisory Bulletin 001 and Advisory Circular 001 was issued 24 December 2003 – States who have yet to do so to develop appropriate regulations and guidance material utilizing COSCAP material – COSCAP-NA to continue to follow up on implementation and advise the 3 rd NARAST Meeting COSCAP North Asia Cooperative Development of Operational Safety & Continuing Airworthiness Programme

28 NARAST Report – DP5 Statement of Work: Operators should have SOPs/training manual. This manual will address: use of checklists, what each person’s responsibilities are, use of available equipment, and expected procedures to be used during preflight, taxi, take-off, climb, cruise, descent, approach, missed approach, landing, taxi and parking. 29 NARAST Report – DP5 SOPs (Rec. 32.4) – COSCAP-NA Advisory Bulletin 002 and Advisory Circular 002 was forwarded to States on 8 th January 2004. States to adapt guidance material and issue as required – The Meeting agreed timely approval of SOPs was important to ensure they were current – CTA to review the issue of SOP approval with States to determine if any guidance material could be developed to expedite the approval process COSCAP North Asia Cooperative Development of Operational Safety & Continuing Airworthiness Programme

30 NARAST Report – DP5 Statement of Work: Identify the means by which all flight crews can fly an appropriate stabilized vertical path to the runway end, for all instrument approach procedures, thereby reducing the possibility of a controlled flight into terrain accident. The Plan will take advantage of existing aircraft capabilities to improve approach and landing safety to the maximum extent practical. 31 NARAST Report – DP5 PAI (Rec. 33.1, 33.2, 33.3, 33.4, 33.5, 33.6) – COSCAP-NA to revise the draft AC on CANPA to include a provision for aircraft equipped with VNAV equipment – Revised draft to be forwarded to NARAST Members for further review – Draft AC related to VNAV to be incorporated into CANPA above without reference to obstacle issues COSCAP North Asia Cooperative Development of Operational Safety & Continuing Airworthiness Programme

33 NARAST Report – DP5 Statement of Work: Develop and implement a mutually agreed upon methodology to use de- identified Flight Data Analysis Programme (FDA) - Flight Operations and Quality Assurance (FOQA), and Aviation Safety Action Partnership (ASAP) information for the purpose of proactively identifying safety related issues and corrective actions. 35 NARAST Report – DP5 Statement of Work: CRM training, standard operating procedures (SOPs), situational awareness, and CFIT prevention are closely linked. This project will reduce CFIT accidents by promoting comprehensive SOPs as a key element of every air operator CRM training program COSCAP North Asia Cooperative Development of Operational Safety & Continuing Airworthiness Programme

36 NARAST Report – DP5 CRM/SOPs (Rec. 35.1, 35.2) – Draft Advisory Bulletin and Advisory Circular on incorporation of CRM into SOPs were reviewed at the NARAST Meeting – COSCAP-NA to issue amendment to Advisory Circular 002 incorporating any comments provided to CTA by 15 May 2004 COSCAP North Asia Cooperative Development of Operational Safety & Continuing Airworthiness Programme

37 NARAST Report – DP5 Statement of Work: Controlled Flight Into Terrain (CFIT) - accidents are the leading cause of commercial aviation equipment loss and fatalities, worldwide. CFIT accidents could be substantially reduced if all air operators developed CFIT prevention training and procedures to be added to their approved training curriculums stressing position awareness and escape maneuvers in the event of a terrain warning indication. 38 NARAST Report – DP5 CFIT Training (Rec. 36.2) – States reviewed draft Advisory Bulletin on CFIT and any additional comments to be provided by 15 May 2004 – ICAO OPS/AIR section requested to provide comments – COSCAP-NA to issue one document consolidating ALAR/CFIT into one Bulletin COSCAP North Asia Cooperative Development of Operational Safety & Continuing Airworthiness Programme

39 NARAST Report – DP5 Statement of Work: The purpose of this project is to develop a strategy to promote a safety culture for air operators specifically targeting approach and landing accident reduction (ALAR). Ensure that essential safety information generated by an airplane manufacturer and regulatory authorities is included in company operating manuals and in training programs for pilots and other appropriate employee groups. 40 NARAST Report – DP5 ALAR/Safety Culture (Rec. 37.1– 37.7) – The NARAST reviewed Draft Advisory Bulletin on Promotion of Flight Safety by CEO and found it acceptable – additional comments by 15 May 2004 after which the documents will be finalized – The NARAST reviewed the Advisory Circular on Air Operator Flight Safety Department and found it acceptable – further comments by 15 May 2004 after which it will be finalized COSCAP North Asia Cooperative Development of Operational Safety & Continuing Airworthiness Programme

41 NARAST Report – DP5 ALAR/Safety Culture (Rec. 37.1– 37.7 cont.) – The NARAST reviewed Draft Advisory Circular on Guidance for Air Operators in Establishing a Flight Document System and found it to be acceptable – further comments by 15 May after which the document will be issued. – CTA to develop and issue Advisory Bulletin on Manual Amendment Status on information provided by Boeing and Airbus by November 2004 – Information on other aircraft to be gathered by COSCAP- NA COSCAP North Asia Cooperative Development of Operational Safety & Continuing Airworthiness Programme

42 NARAST Report – DP5 Statement of Work: Ensure that air operators implement syllabi that train and evaluate aircrews on stabilized approaches, unusual attitudes, and upset recoveries. Specific topics related to stabilized approaches should include: crew resource management, go around criteria, approaches with system malfunctions, non- normal conditions, emphasis on basic airmanship, approach briefings, approach and missed approach procedures. 43 NARAST Report – DP5 ALAR Training (Rec. 2.2.1, 2.2.2) – NARAST to develop issue a Bulletin on ALAR to incorporate training aspects to reduce the risks of ALA 2004 – The draft Bulletin on CFIT to be amalgamated into the ALAR guidance material and issued as one Bulletin COSCAP North Asia Cooperative Development of Operational Safety & Continuing Airworthiness Programme

44 NARAST Report – DP5 Statement of Work: To ensure that all airline operators publish and enforce clear, concise, and accurate flight crew standard operating procedures (SOP). These procedures should include expected procedures during pre/post flight and all phases of flight i.e.: checklists, simulator training, PF/PNF duties, transfer of control, automation operation, rushed and/or unstabilized approaches, rejected landings and missed approaches, in-flight pilot icing reporting, and flightcrew coordination. Operator instructors and check pilots should ensure these SOP’s are trained and enforced in their aircrew proficiency and standardization programs COSCAP North Asia Cooperative Development of Operational Safety & Continuing Airworthiness Programme

45 NARAST Report – DP5 Lost of Control SOPs (Rec. 2.3.1, 2.3.2, 2.4.1) – FAA to review NARAST AC on SOPs to ensure it contains Loss of Control – CTA to finalize the amendment to SOP Advisory Circular once it is confirmed Loss of Control is incoporated – Loss of Control - Risk Assessment and Management is still being reviewed by FAA/JAA so NARAST will await the results of this review COSCAP North Asia Cooperative Development of Operational Safety & Continuing Airworthiness Programme

46 NARAST Report – DP5 Statement of Work: SE28/AP3.05 The purpose of this project is to ensure that essential safety information and operational procedures generated by airplane manufacturers are included in companies' operating manuals, training programs for pilots and other appropriate employee groups, in daily operations. 47 NARAST Report – DP5 Safety Information (Rec. 2.5.1) – NARAST has already developed a draft Advisory Circular on Flight Safety Document System – CTA to review the draft document and ensure it satisfies the requirement of SE28/AP3.05 COSCAP North Asia Cooperative Development of Operational Safety & Continuing Airworthiness Programme

48 NARAST Report – DP5 Statement of Work: Operators, in collaboration with pilot associations, should ensure their training and qualification processes utilize information from programs such as FOQA, AQP, and ASAP to assist in assuring pilot proficiency. 49 NARAST Report – DP5 Loss of Control (Rec. 2.6.1) – The NARAST draft FDA Advisory Circular highlights the need to utilize the information from Flight Data Anaysis to verify and optimize training. CTA to verify that the draft AC includes aspects as outlined in SE29. COSCAP North Asia Cooperative Development of Operational Safety & Continuing Airworthiness Programme

50 NARAST Report – DP5 Formation of NASTs: Some Member States have indicate an interest in the establishment of a National Aviation Safety Team (NAST). In addition to assisting the implementation of safety enhancements from NARAST, that will require inputs from different functional areas, the NAST could be a focal point for safety issues that are particular to only that State. In addition, the NAST could also identify safety issues for discussion by NARAST. CTA provided draft terms of reference for review of the NARAST members. 51 NARAST Report – DP5 National Aviation Safety Teams (Rec. 3.1) – States should develop National Aviation Safety Teams to coordinate with NARAST, to review/discuss safety related matters not of a regional nature and to identify safety related matters to be discussed by NARAST COSCAP North Asia Cooperative Development of Operational Safety & Continuing Airworthiness Programme

52 NARAST Report – DP5 3 rd NARAST Meeting - Most of the major flight operations issues have been discussed. Considerable work remains for COSCAP-NA and Member States concerning the implementation of these safety enhancements. Implementation of some of the safety enhancements being developed will also require the coordination and input from Air Traffic Management. Some additional safety enhancements such as Runway Incursions, CFIT, etc. will require the review of both Flight Operations Specialist and Air Traffic Management Specialists. 53 NARAST Report – DP5 The 3rd NARAST Meeting to include both Flight Operations and ATM specialist The CAST/JSSI kindly requested to provide ATM support for the 3rd NARAST Meeting In discussions with donors and in coordination with SARAST/SEARAST it was suggested that the target date for the 3 rd NARAST meeting be 19-21 January 2005 COSCAP North Asia Cooperative Development of Operational Safety & Continuing Airworthiness Programme

54 ICAO Language Requirements – DP6 – Pilots issued a licence after March 2004 must demonstrate ability to speak required language - ATC required to demonstrate proficiency in language used for communication - Pilots and ATC, by March 2008, must demonstrate proficiency to the level specified in Annex 1 - As of March 2008 pilots and ATC must demonstrate proficiency at intervals COSCAP North Asia Cooperative Development of Operational Safety & Continuing Airworthiness Programme

55 ICAO Language Requirements – DP6 - Cooperative arrangement was discussed at the 2 nd COSCAP-NA SC Meeting and it was decided to await further information - ICAO DOC 9835 Manual on Implementation of ICAO Language Proficiency Requirements is now available in Draft - The Manual will serve as a guide to CAAs, airline industry and training organizations on implementation of the SARP COSCAP North Asia Cooperative Development of Operational Safety & Continuing Airworthiness Programme

56 ICAO Language Requirements – DP6 - ICAO will hold an Aviation Language Symposium at Montreal from 1 – 3 September 2004 - The Symposium is to provide information to facilitate implementation of the of the SARP primarily in aviation English language training and testing - Also to provide advise participants with ways to assess the quality and appropriateness of solutions proposed by the language industry COSCAP North Asia Cooperative Development of Operational Safety & Continuing Airworthiness Programme
