3 A recent industry study by the Health Information Network identified five key challenges in implementing palliative care into transitions programs. We will address each and give each panelist a chance to address each in this panel.

4 51.6% of respondents identified timely referrals as a challenge in implementing palliative care in transition programs.  Do you agree that this is the biggest challenge?  What are a few strategies to improve this problem?
