Form H-10 Annual Injection/Disposal Well Monitoring Report
Mary Torres Form H-10 Annual Injection/Disposal Well Monitoring Report This form keeps a history of injection/disposal wells H-10’s began in 1982 It allows all interested parties to view the activity of a well . What type of fluids or gas has been injected or disposed of . It allows the RRC /public to make sure all wells are being operated within the permit(s) issued to P-4 operators.

Answer: You can file if you receive a notification that your form will be due in the near future My P-5 shows my well is inactive but , my well is currently active how can I change status? Why did I receive a certified letter for item #15 It is a Special Condition issued on your permit. It is a permit violation not to complete all 3 columns and may cause a severance/seal

All operators with active injection/disposal permits
Who has to file? All operators with active injection/disposal permits This is how we , RRC keeps track of a well and the stage/status of a well. A H-10 request is mailed to a P-4 operator , even if the well is not drilled. Meaning you only have a “permit” but, as the stages of the well continues to develop , the RRC needs to know and keep track. For a new permit , a well is classified as “permitted or not yet drilled “– a H-10 will show zero’s on item #14. Once it is active a H-10 will be reported with positive volume or gas ( item #14) If a well is temporarily abandoned an H-10 is still required, because you still have an active permit If a well has been plugged a W-3 is required , once W-3 is submitted and our system has been updated , an H-10 will no longer be required When a permit is canceled , an H-10 will no longer be required.

Hard copy forms are no longer provided
NOTIFICATION A listing is mailed 3 months in advance advising operators of upcoming H-10’s that will be due Hard copy forms are no longer provided Wells are staggered by field so that not all H-10 ‘s are mailed in at the same time. Imagine all H-10’s submitted on the month of January ! This listing is also so that operators have plenty of time to gather all their data. Hard copy forms can be printed from our website/query. Although still accepted , hard copies run a risk of not arriving on time to avoid a severance/seal. Which carries a non refundable fee of $ per severance/seal. If a hard copy form is rejected it is considered not here and again can cause a severance/ seal with a fee If you don’t have a computer to download a hard copy, you may need to go to a public library or one of our district offices to print your form(s)

How to file a correct and acceptable Form
A form is acceptable when: Hard copy : All header data is correct –uic number, district number, lease/gas ID, well number, well type, effective and due dates, field and lease names, the 12 months and years on item 12 are correct. Also, the pressures are within what the permit states and left blank when no volumes are reported. All months on item #14 are reported with amount of fluid/gas injected or a zero is placed if well is inactive for each month(s). All active well must equal 100% of fluid(s) injected/disposed. If a special condition is on your permit . Data is to be reported on item #15 and ALL 3 COLUMNS must to be complete for form to be accepted.

Online filing Advantages hard copy is still acceptable.
Expedites the process RRC receives it within seconds and immediately places a receive date Can be filed 24/7 Violations are resolved within 24 hrs. This avoids severances and seals. No question as to when it was filed . A receive date is immediately posted. When a form is delinquent, you can file and can be viewed and considered received within a few seconds. Knowing you can file 24/7 out of the comfort of your home or any place where there is Wifi, is a big plus. No need for postage , making sure we (RRC) have received it, was the form signed , is the form for the correct well- because the UIC number stays with the well for the life time of the well. If you key the wrong UIC number and the rest doesn’t match up to that UIC number, the system will not let you continue to file. Hard copies , you can make any or many mistakes and you won’t know until we mail you a letter. This is risky, because the form will be considered not received and may cause you a severance/seal . hard copy is still acceptable.

Getting Started RRC online filing page
If assigning filing rights log in with security administrator userID and password If filing H-10’s enter your userID and password If you are the security administrator for your company and ONLY assign filing rights, you only need one UserID and One password but, if you are the security administrator AND you are also the filer, you will NEED TWO DIFFERENT userids and two different passwords. When you’re going to assign someone else to do the filing, use your Security Administrator userid and password, create a person and give them rights to file H-10’s. If you are going to do the filing, use your FILER userid and your FILER password but, you must first update yourself as a filer and give yourself the rights to file.

For User Id enter your address For password enter your user ID If you forget your userid and or password. us , use this screen and we will respond to you and assist you.

Security Administrator assigns filing rights to filers.
H-10 External Filer to file one H-10 at a time. EDI Upload to upload spreadsheets Security Administrators– here is where you assign filing rights for someone or for yourself. Please click on the correct method of filing. EDI is for several wells at a time- a spreadsheet.

These are the filing rights you have access to.
For H-10’s click on “H-10 Filing System” If you want to check to see if you have been given the rights to file , this is the screen that lets you know what you are authorized to file. Notice that H-10 filing system is a choice. Click on that choice and you can start to file.

The top section is to file one H-10 at a time.
The middle section is to view all H-10’s due current and delinquent. *very useful* The last section is to make any corrections. The first section is for filing one form only. Key in the UIC number or the API number only , the next screen is the H-10 you requested. The middle section is very useful. By going to DISTRICT and arrowing down to STATEWIDE click search , this will show you all the wells that are DELINQUENT or are CURRENTLY due. It will not let you view or file for wells that you have not received a notification on. The bottom section is to make corrections. From 2007 to current. Please note that if you give this screen too much information , it will not open up the form. The best data you can key in is the UIC number and due date (if applicable) ONLY. If you received a Notice to file for a particular well. It will be on this screen ready to file . If you did not receive a notice , the H-10 will not be found .

By choosing “statewide” you can see what is due and when.
Click on “file” to automatically open up the form and start to file. Here is an example of STATEWIDE. This is very helpful for consultants/ agents and your new employees.

How to complete H-10 form For inactive wells, click on No Volume to Report This will automatically populate item 14 with zero’s. No other data is required. No alpha Process/submit By clicking on “NO VOLUME TO REPORT” you are indicating there was no volume injected, this will populate item 14 with zero’s. Scroll down to the bottom to process, next screen to submit. The system will not allow a form to be submitted with data on items 16-20, for inactive wells. It will automatically delete data.

Pressures Active wells must report Average and Maximum pressures on item #13 For negative or vacuum pressure zero’s are required, do not leave blank This data usually comes from the pumper. The only time the form can be left blank is when there is no volume reported.

VOLUMES Item #14 Active wells must report amount of BBL’S/liquid or MCF/gas injected each month Inactive wells require a zero for each month This also usually comes from the pumper. For hard copies—please do not indicate the type of fluid or the percentage of fluid/gas if the well was inactive for the months and years on this reporting cycle. This could lead to enforcement-severance/seal.

Special Condition Pressure between the tubing and casing.
Report on item# 15 all 3 columns need to be completed Required if “special condition” on permit Not a requirement for all wells. Please check your permit. If required, indicate the times per month and both the min and max pressure. This is the pressure between the tubing and the casing. The backside. This is a MUST if it is on your permit.

System will not let you submit if red error message appears.
Once corrected continue to process and submit. You will need to correct any errors before you can submit. For hardcopies - this is a reject and you will only know when a letter is mailed out to you. If you are trying to avoid a severance/seal deadline the hard copy form will be considered not here and returned to you with a reject letter.

It lets you know the form was successfully submitted. If you don’t see this message , your form has not been received. Or check the H-10 query.

Intervals/Zones Active wells must report the zones where they are actually injecting into. Not the permitted zones. For types 1 (disposal) 2 and 3 (injection) it’s ok to inject 100’ above the top permitted zone or 100’ below the bottom permitted zone. It is also ok for both to be 100’ above or 100’ below the authorized permitted zones. If injecting in different zones, shut well in and/or amend your permit. All permitted injection/disposal wells are permitted with authorized intervals Ex: top zone bottom zone the ACTUAL injection zones can be any where between top to 4310 bottom Can be injecting 100’ above or 100’ below the permitted zones A W-2 does not override permitted zones.

Packer Item # 17 requires that you report where the
packer is located, not where it is permitted. Some permits/wells have packer exceptions. For type 1 (disposal) packer can be 100’ above the top permitted zone. Types 2 and 3 (injection) ok to be 300’ above top permitted zone. If you need a packer exception, you will need to write a letter explaining why this is needed. For disposal wells: Type 1, the packer can not be higher than 100’ above the AUTHORIZED PERMITTED top zone - period. For injection wells. Types 2 and 3, the packer may be set 300’ above the top permitted zone. You will not receive a packer violation letter.

W-2/G-1 Data for H-10’s When injecting into different depth, that is still within the authorized permitted zone Is required when converting well from producing to injection/disposal How W-2’s/G-1’s convert an inactive well to a currently active well for HB 2259 (P-5) When a new injection/disposal well is activated, a pressure test is required before injection. A W-2/G1 is required within 30 days of activity. Anytime injection/disposal zones change – but still within the permit- a W-2/G1 is required. A W-2/G1 is required to re-classify a well. HB2259: The last H-10 on file determines the status of a well. For those that are filed with 12 months with no activity, these are classified as temporarily abandoned or shut in wells This usually effects your P-5. An H-10 is only filed annually. Partial H-10’s are not accepted . If a well is CURRENTLY active and your H-10 is NOT currently due, you will need to file a CURRENT W-2/G1 FOR WELL RECORD ONLY . On remarks indicate well is currently active. This will override the P-5, BUT for one year only.

Produced Fluids Item #18 requires that you report where the injection/disposal fluids are coming from Are fluids purchased or from your own wells This is if the fluids/gas injected are being trucked in.

Injection Through Item #19 check the box if you are going through tubing or casing. Casing requires an exception. Approved by the RRC.

Reporting types of fluids
On October 24, 2011 Form H-10 was revised. On item #20 “types of fluid injected during reporting cycle”. This means the 12 months printed on item #12, not what would’ve been injected All fluids/gas injected must equal 100%. Hard copy – old forms are no longer acceptable. If on item # 20 there isn’t a “ % “ sign, you are using the old form. The system will not accept it. You will not be able to find the Old form on our website to print and file. You will need the UIC or API number to find the H-10 on our website/queries. This will help you located the correct pre-print form so that the percentage of fluid/gas can be reported

Reporting types of fluids
If well is inactive, items # 16 through 20 are left blank. Reporting online for item # 20 please use the drop down arrow to view additional fluids. For hard copy, if you don’t see the fluid type you are injecting please, use “other” and specify. Hard copies– please don’t write 100% (item #20) of any fluid/ gas type if well is not active . This will cause a delay and your from will be rejected , you could run into a severance/seal. The form will be returned and considered not received. Please don’t place a check mark or an “X” to indicate type of fluid/gas that would’ve been injected, this too will reject your form. Please don’t write any alpha EX: plugged , cancelled , temp. abandoned , shut in, producer or anything else on the form. If you just took over from another operator and it is half way through the reporting cycle and you can’t get the data from the previous operator, place zero’s on item #14 for the months you didn’t operate the well. We know who the previous operator was and when the new operator took over. We can check the P-4.

Violations/Rejects H-10’s are automatically checked by our system for violation or rejects. If required, the system places them in a queue for further review. A notification is mailed to the P-4 operator. You can Correct forms online or mail in a corrected hard copy. Both online and hard copy H-10 forms are automatically checked for violations. A violation means you are operating your well or wells out of permit compliance. A violation letter is mail to the P-4 operator advising them of the violation(s). Please make sure you resolve your violation before it goes into a severance or seal.

Violations/Rejects If no response within 30 days, a certified letter is issued, 30 days after that if violation/reject is still active a severance/seal is issued. $ reconnect fee is required If permit is being amended please, let me know. A 60 day hold will be placed. Please notify me when you are amending your permit. This is so that the violation can be placed on HOLD before it goes into enforcement. You can call or me. If amending a permit, once a severance or seal is issued you will not be able to operate the well until the request for an amended permit is approved or denied. Placing the well on hold allows you to continue to operator the well, as long as the well is being operated within the existing permit requirements.

Well missing from the request list ?
Please contact Mary Torres or Include UIC # or API #. Research will be done, most likely a current W-2 or G-1 is required and/or a root record will need to be created, this takes about 3 or 4 weeks. When you can’t find your form please, let me know. I can research to find out why you are unable to locate the form.

Filing hardcopy: Once received, form will be outsourced takes 5 to 6 days for turnaround Effects time sensitive severances/seals Filing on outdated forms are returned and are considered not yet received Please make sure you give the mail and us (RRC) enough time to show your form as received. This is especially important when your trying to get your forms in before a severance/seal is issued. Using a overnite service does not guarantee the form will be received on time or accepted when trying to avoid a severance. Remember…it takes the outsource company 5 to 6 days for a hardcopy turn around.
