PopMedNet and ADVANCE Jon Puro, MHA, Research Information Architect, OCHIN (Portland, OR)

ADVANCE CDRN quick overview ADVANCE: “Accelerating Data Value Across a National Community Health Center Network” Led by OCHIN’s Practice-Based Research Network (PBRN) in partnership with Health Choice Network (HCN) and Fenway Health Systems  PI: Dr. Jen Devoe (OHSU and OCHIN); co-PI: Jon Puro (OCHIN)  Outpatient clinical data from 111+ FQHC health systems across 22 states  A “community laboratory” of FQHCs  Longitudinal clinical data on 2.8 million+ patients  Partnerships with health plans and hospitals  Community-level data 2

OCHIN is a nonprofit health care innovation center designed to promote access to quality, affordable health care for all. Who Is OCHIN? One of the nation’s largest and most successful Health Information Networks In 18 states, coast to coast Touching over 4,500 physicians

ADVANCE Network: OCHIN, Health Choice Network, Fenway Health

ADVANCE infrastructure ADVANCE:  Based at OCHIN  One central CDM in which we store all data from ADVANCE partners  OCHIN hosts central Epic system (Clarity, Crystal Reports), SQL Server, SAS  These only work for OCHIN data (Epic), not HCN (Intergy) or Fenway (Centricity)  OCHIN/HCN “Acuere” QI system for data access: HL7 data ADVANCE needs:  Click-and-drag OLAP interface;  For non-technical users;  To facilitate fast and efficient prep-to-research and cohort discovery. 5

Experience setting up PopMedNet PopMedNet installation was fast and easy Simple usage so far (query/response) Currently installed on one workstation (mine) No special permission needed to install, but:  For wider user audience, approval process will be needed for: Installing software and Granting access and Controlling levels of access: user-only vs. admin- level. 6

Use Cases: ADVANCE and PCORnet Internal ADVANCE querying (“intra-network”) vs. external PCORnet querying (“inter-network”):  Internal ADVANCE prep-to-research queries: ADVANCE-affiliated researchers doing prep-to-research queries Allow full access to data and results ADVANCE admin creates logins and manages accounts  PCORnet queries sent to ADVANCE and other CDRNs: Queries come from non-ADVANCE researchers against ADVANCE CDM; Queries must first be reviewed and approved, then run “manually”; Security administration based at PCORnet DRN. 7

Functionality needed to support Use Cases Intra-network:  Click-and-drag OLAP cohort discovery interface;  Automated query-response;  ADVANCE-level security administration Inter-network:  Click-and-drag OLAP cohort discovery interface;  “Manual” response still required due to governance;  For efficient manual process, use PMN to: –Display query syntax; –Run the query against CDM via SAS or SQL; –Create output data files; –Deliver final output. 8

Summary Allow for multiple concurrent users Different levels of account security (user vs. admin) Internal query/response could be “live” Interfacing with SAS and SQL:  Show query syntax in PMN;  Run the query against CDM;  Create output data files  Deliver final output (or at least have an interface to SAS to minimize manual steps). Create on-boarding process, incl. user training (videos) 9
