7. Billing Student Fees allow us to discount our services We are happy to bill most insurances If we are aware that a student’s private insurance does not reimburse us for our services, we will assist the student in finding a clinic that does (typically, a University clinic within our building). The student is responsible for any charges not covered by their insurance. Certain insurances (Kaiser) provide no coverage outside of their coverage area.

9. Immunization Requirement Two MMR’s (Measles, Mumps, and Rubella) are REQUIRED Fill out the “Proof of Immunity” form and return to Student Health Center before September 30 th Only religious or medical exemptions are allowed

10. “I can’t find the dates of my MMR” For Utah students, the dates are usually available through your high school If you are sure you have had the immunizations, blood titers can be drawn – However, if you are negative, you will then need the vaccination. – Insurance may cover the cost of vaccinations
