Teacher Evaluation Forms (Green) 3 per girl 2 Core/Team Teachers+1 Choice Teacher Returned in Sealed Envelopes by May 18 to Coach Murphy C272 or Coach Burton C163 Team Interview (Purple) Returned by May 18 to Coach Murphy C272 or Coach Burton C163 Tryout Scoring Sheet (Yellow) Tryout Forms

All forms must be completed accurately before participation Athletic Participation Form Requires Birth Certificate and Notarization Eligibility and Information Form Physical Form-FHSAA Preparticipation Evaluation Completed by Doctor Consent and Release from Liability Consent and Release for Concussion and Heat-Related Illness Athletic Forms

Practices for Tryouts Monday-Wednesday 3:10-5:00 May 18-20 Cannot Participate without all Athletic Forms Submitted Cannot Participate without both sealed Teacher Evaluation forms submitted Tryout Numbers Each girl will be given a number at random Must have and wear their number for every practice and tryouts Practice Procedures

Attire Black Athletic Shorts-so spandex or spanx White T-shirt Tennis Shoes Hair Pulled out of Face No Jewelry No Cheer Bows Bring your own water No Food or Gum at practice No Phone Usage between 3:10-5:00 Practice Procedures

Final Tryouts Thursday 3:10-5:00 May 21 Tryout in groups of 4 (numerical order) Closed Tryout-No one permitted except for immediate participants Attire Same as Practices Results Posted Friday Morning in Gym Windows 3 Impartial Judges not affiliated with Seven Springs Middle School Cheer Squad Tryouts
