Summary of personal, educational, and employment information Used to help prepare for filling out a job application and/or a resume They are NEVER given to the employer! They contain 4 primary sections ◦ Identification ◦ Educational Background ◦ Employment History ◦ References

Employers ask for information about job candidates in various ways Pay special attention to any instructions when reading an ad Some employers will only accept information electronically Some will not accept phone calls about openings Some require additional material like work samples or references If you don’t follow the instructions, you won’t get the job!

Used by employers to collect personal, educational, and employment information from a job applicant The information helps employers to sort out the best-qualified persons for the job Your personal data sheet will very helpful when filling out the job application The type of application will differ from company to company

READ the form first Only fill-in information that pertains to YOU Paper Applications MUST be NEAT with NO ERRORS HAND-PRINT your information Do not use nicknames SIGN the form where it indicates NEVER put false information Check your SPELLING Follow ALL submitting instructions

Some employers collect applications for positions that begin on a certain date Some employers may have no jobs at the moment, but will keep applications on file for future use Make copies and write the information down on your job lead card If after a week, you haven't heard from them, it’s OK to contact and show that you are eager for the job
