PHOS-ISTOS D EVELOPMENT OF BIOPHOTONIC DEVICE BASED ON FLEXIBLE LIGHT EMITTING TEXTILE DEDICATED TO THE MONITORING AND TREATMENT FOR DERMATOLOGIC DISEASES AND CARCINOMA
2
C ONTEXT Actinic Keratosis (AK) Cutaneous carcinoma in situ Scaly erythematous plaque Caused by sun damage Epidemiology: In Europe a prevalence of 15 % in men and 6 % in women has been documented Over the age of 70 years, 34 % of males and 18 % of females were found to have AKs Evolution 10% risk to develop into invasive Squamous Cell Carcinoma 2 Localization on sun-exposed areas
3
C ONTEXT Photodynamic therapy (PDT) for Actinic Keratosis Destruction of the AK cells by the interaction between three elements: Photosensitizer: Protoporphyrin IX Generated from a topical photosensitizing agent Selectively accumulated in the target cells Light of appropriate wavelength Oxygen in sufficient quantities in the target cells 3
4
C ONTEXT Photodynamic therapy (PDT) for Actinic Keratosis Benefits Cure rates between 69 and 93 % Excellent cosmetic results: no scare Drawbacks Important variation in cure rates Perfectible Very painful therapy Half a day equipment occupancy per patient Cost and limited availability of equipment (only available in specialized environment) 4

O BJECTIVES 5 Objective: To increase the efficiency of treatment Solutions proposed by Phos-Istos: To ensure illumination homogeneity using a flexible light emitting textile To determine in real time the optimal treatment parameters by monitoring the PDT reaction Conventional rigid LED panels: Flexible light emitting textile proposed by Phos-Istos:
6
O BJECTIVES 6 Objective: To manage patient pain Solutions proposed by Phos-Istos: To ensure oxygen in sufficient quantities using light fractionation (succession of illumination and rest periods) To provide a cooling effect on the affected area Fractionated illumination proposed by Phos-Istos: Conventional continuous illumination: ON t OFF t ON Reoxygenation during rest periods
7
O BJECTIVES 7 Objective: To extend the use of PDT Solutions proposed by Phos-Istos: To miniaturize the system for ambulatory use To limit the cost of the ambulatory system Conventional PDT systems: Ambulatory system proposed by Phos-Istos:
8
E VALUATION 8 Comparative clinical evaluation of the light emitting textile for AK Efficacy assessments: Complete (or not) disappearance of lesions 3 months after treatment Safety assessments Pain assessment during treatment Assessment of adverse effects (erythema, crusting…) 1-3 days after treatment. PDT using conventional red light-emitting diode light PDT using the light emitting textile
