Equipment - Supplies Equipment food prep, yogurt pumps, smoothie machines, cart, kiosk, wagon, food bar, trash cans, recycling bins, cleaning supplies, hand sanitizing dispensers Small fitness equipment (e.g., jump ropes, traffic cones, stability balls, Frisbees and Large fitness equipment, Tracking/Monitoring equipment, Video game consoles and software CDs, DVDs (Xbox Kinect, PS3/Playstation, Nintendo Wii) Both Permanent signs, menu boards, bulletin boards, coolers, insulated bags, refrigerators or freezers

Funding Use – Equipment and Services Mission - We plan to use our funding to influence and implement healthier food options at the regular events we host each year. Commercial grade smoothie blender – rewards or incentives for class events Portable serving tables and containers – hold healthy food and drinks at events Serving and storage containers – to distribute items for our nutrition plays Fresh and frozen fruit, Greek yogurt, smoothie mix – to make smoothies Smoothie parties will be available as incentives for classes meeting their walking goals Fresh fruit and dairy – breakfast and lunch programs and special events Bulletin boards, educational posters, bookmarks, stickers – purchased to reinforce healthy eating concepts. Equipment and healthy food items – purchased for prizes and incentives to promote fitness and wellness at our annual Healthy Holbrook Family Fun and Fitness Night. Plan for the End in Mind …..Include Success Notes: 18-19, 24-25 Explain how your school will continue with your Healthy Eating /Physical Activity Play after funding is exhausted. Our Fuel Up Oatmeal Fridays were such a success last year that our cafeteria has built it into the regular cafeteria breakfast cycle. The cafeteria will work with us by supplying and featuring new items we have introduced – whole grain bagels, low fat cheeses, veggies, toppings of dried fruit, nuts or peanut butter. With our input from our student surveys last year our cafeteria will incorporate foods into the menu well liked by our students.

Fuel Up Friday Nutrition Play Build a Better Breakfast Fuel Up Friday Oatmeal Breakfast Bar What - Hot oatmeal with healthy toppings including fresh and frozen fruits, low fat and fat free milk, yogurt and dried fruits and nuts. Where – In the cafeteria When – Once a month before school Cost – Breakfast is available free of charge to all students How – Oatmeal is prepared by cafeteria staff with the Fuel Up Team providing themed toppings Who – Fuel Up Advisors select oatmeal toppings and student ambassadors assist with preparation, serving and program promotion.

Healthy Eating/Physical Activity Play Sustainable and Specific 1. Fat-free and low-fat dairy – will be featured in our school wide events and lunch salad bar. Greek yogurt, low-fat cheese varieties, low fat white, strawberry, chocolate milk, yogurt and fruit for smoothies 2. Whole grains – used as options for our school wide events. Options include granola, whole grain oatmeal, whole grain tortillas, whole grain cereal, breads, muffins and bagels 3. Fruits and vegetables –events include fresh and frozen fruits, juices w/o added sugar, dried fruits and fruit leathers. Vegetables such as zucchini, carrots, peppers, coconut, celery, tomatoes or breakfast, lunch and served at school wide events. Fresh fruit smoothies as class awards. *Lean meats, poultry, fish, beans, eggs and nuts – turkey dogs/chicken *Low in saturated fats, trans fats, cholesterol, salt (sodium) and added sugars, water, baked chips, low fat ranch Application Section 4 & 5

Consider Yearly Funding Limits $2000 for each play – you may just apply for one $500 Kickoff and Promotional Activities - temporary signage, posters, or printed material to promote Fuel Up $300 Internal Staff Stipends or (incentives-gift cards) $400 If utilizing outside professionals $200 Nutrition education materials $200 Incentives/Rewards – Nutrition & Physical Activity Food for taste test activities - 60 cents per student
