“Top 5” things you should know about 1. All providers and staff are encouraged to report safety events 2. Report any event—or hazard—that has or could result in patient harm: Don’t need to wait for something bad to happen…report things with potential for harm Adverse clinical events Things that make you go “Hmmm…” Unusual or undesirable chain of events Inefficiencies/problems which interfere with optimal care delivery

“Top 5” things you should know about 4. It’s EASY to report! 1: Use your University uNID and password to access the system 2: Use the search field or select the icon that best fits the incident 3: Complete the form fields 4: Click “Submit” when complete

Downloading RL6:Mobile Download RL6:Mobile : App Store (Apple) Play Store (Android) Select “Manual Entry” and configure your device by typing this URL :

Select “Manual Entry” Type in the URL

Using RL6:Mobile Click “Connect” Login using your uID and password

Select: New General Event Submission Provide requested information

Finishing Up “Submit” Will get a file ID number for future reference if needed
