3. Information on Request for Response 3 Click on Complete on-line registration and RFR questionnaire.

4. Login 4 Enter your Username and Password or click on link that says, “please register by clicking here” to create an Username and Password. Click “Submit.”

5. 55 Applicant Information The fields marked with * are required. When are you finished entering click “Next Page”.

6. Select region/towns and budget levels 66 Select a region from the dropdown menu. Then click, “Next Page”.

7. Select region/town and budget levels 77 Click on towns you serve. Then, the Total Funding, Average Providers Served/Month,and Averaged Families Served/Month will appear in boxes.

8. Saved Budgets 88 Once you have created a budget for a region, it will appear on this screen. You can view and edit from here.

9. Part II- Budget Detail 99 Make sure to fill in # of staff, FTE and total for each line item. Then click “Save and Continue.”

10. Budget Narrative 10 You must type in a narrative for every line item that has a total in the budget.

11. Purchase of Service Budget 11 Make sure to fill in FTE and amount for each line item. Then click, “Save and Continue.”

12. Purchase of Service-Program Budget Narrative 12 You must type in a position title, FTE and cost justification for every line item that has a total in the budget. Then click, “Save and Continue.”

13. Print All Budgets and/or Print Budget Summary 13 You can edit your budget any time before the deadline. The Print All Budgets button will print the entire application. The Print Budget Summary will only print the budget pages.
