6. 6 Mandatory Forms In order for a student-athlete to participate in any practice, scrimmage, or game, they must have all mandatory Fulton County forms on file with both the Head Varsity Baseball Coach and the Athletic Department. The following forms must be completed and are valid for one (1) calendar year: – Physical (must be on Fulton County form) – Fulton Athletic Insurance Consent – Concussion Form – Participation Form The above forms can be found at http://www.northviewbaseball.com Turn these forms into Coach Wilkens by October 30 th. – Your child cannot participate in pre-season conditioning until each of these forms is received. Fulton County requires these forms, not the Dugout Club
