3 Goal: Ensure education of our senior community members, promoting their independence and a higher quality of life. Objectives:  To develop monthly educational seminars to inform the community of available services to seniors  To ensure distribution of information resources throughout the community regarding services available to seniors Goal: Enhance and improve the functional ability of our patients, resulting in greater autonomy in activities of daily living. Objectives:  To complete 100 in home assessments fall risk assessment and implement appropriate interventions to decrease falls by 50%  To assess current and best practices for home delivered food in emergency weather conditions to determine if there is a need for additional services within one year Midwest Senior Advocates

4 Goal: Increase health promotion practices amongst seniors Objectives:  To educate and encourage senior immunizations increasing vaccination rates among senior citizens by 30%  To increase health awareness of our patients and their families by holding classes of 10 patients on a weekly basis.  To increase the health awareness of our patients and their families by providing classes educating them on disease processes and management of those diseases. Goal: Maintain a holistic and interdisciplinary approach to patient care, by treating the whole person and not simply managing symptoms. Objectives:  To develop and initiate a “Senior Buddy” program to increase socialization within one year.  To collaborate with local religious institutions to determine the need for transportation services to and from religious services within one year.  To develop and initiate a program to connect seniors to volunteer opportunities in the community within one year. 6 The events coordinator will work along with the nurse educators and other team members to evaluate the educational needs of the elderly community. They will direct, coordinate and plan screenings and educational events for the organization. Secretary Events Coordinator Will oversee the functioning of the overall facility and ensure that the mission and values of the organization are being carried out efficiently. This individual is also responsible for maintaining the established ethical and legal standards for the workplace. Plays a role in attending meetings with other healthcare organizations to build and maintain partnerships with this organization. The secretary interacts with community members over the phone and in person to answer questions regarding educational events and screenings Midwest Senior Advocates

7 The nurse educator is responsible for developing an educational plan for the elderly on topics of disease process, nutrition, medications and physical exercise. These educators may work with a group of community members or may schedule one on one time to cover specific issues. They must integrate evidenced based practice into the educational plans and work closely with the event coordinator to schedule group educational services. Interested? 8 The advertising and marketing director develops eye-catching materials to appeal to the elderly and forms partnerships with other healthcare organizations to better serve area seniors. The Budget Coordinator will manage all incoming financial resources and allocate appropriately throughout the organization. Additional responsibilities include management of accounts payable, and to evaluate and advise on cost containment issues within the organization.
