4 Treatment Sequence n History, Exam, Primary impressions n MMR, Face bow n Treatment plan, design, informed consent n Tooth modification, secondary impressions n Casting try-in n MMR, shade selection n Wax try-in n Primary issue n Secondary issue n Fluoride treatment/ Exam/ Recall

5 Primary Impressions n Aim: To obtain an accurate models of the patients maxillary and mandibular oral tissues.

7 Facebow Record n Adjustable articulators allow the cast of the maxillary denture to be located accurately in relation to the mandibular condyles and to a horizontal plane of reference (frankfurt plane). The Facebow records this information clinically and transfers this relationship to an articulator. n The mandible moves along an arc of rotation as it opens and closes. The centre of rotation is in the condyle. The distance from the centre of rotation of the arc to the teeth is relevant in developing the occlusion

8 MMR: maxillo-mandibular relationship n Aim: –relate the upper cast to the lower cast in the correct anatomical relationship. –Relate the upper cast to the condyles in the correct relationship

9 How do you do this? n Hand articulate n If you cannot hand articulate you will need to use base plates with wax rims. n (The casts are then articulated)

10 Surveying and denture design n The articulated casts are surveyed before the denture is designed.

11 Special trays n Once the design has been finalised special trays are constructed.

12 Tooth modification n Based on the denture design carry out any tooth modification: –Rest seat preparation –lowering of survey lines –Guideplanes

13 Secondary impression n Once the tooth modification has been carried out a new impression is taken using the special tray.

14 Retake MMR n The new cast needs to be re-articulated against the existing lower cast. n If the casts cannot be hand articulated new baseplates and wax rims will be required.

15 Set teeth and wax-up n Once the casts have been re-articulated the teeth are set on the baseplate. n The wax contours are finalised. n The denture is now ready to process…...

16 Process denture n Heat cure acrylic - invest and pack n Cold cure acrylic - construct key

17 Issue n Trim and polish denture carefully to maintain contours and maintain contact. n Polish with pumice and then with dentishine. n The denture is now ready to issue…...
