Making it Real Markers for change Information and Advice. Having the information I need, when I need it. I have the information and support I need in order to remain as independent as possible. I have access to easy to understand information about care and support which is consistent, accurate, accessible and up to date. I can speak to people who know something about care and support and can make things happen. I have help to make informed choices if I need and want it. I know where to get information about what is going on in my community

Definition of wellbeing Personal dignity (including treatment of the individual with respect); Physical and mental health and emotional wellbeing; Protection from abuse and neglect; Control by the individual over day-to-day life (including over care and support provided and the way it is provided); Participation in work, education, training or recreation; Social and economic wellbeing; Domestic, family and personal relationships; Suitability of living accommodation; The individual’s contribution to society

Asset /strength based assessment 'Asset-based' assessments or 'strength-based approaches' offer a new way of doing holistic person-centered assessments. Assessments of need often define needs in terms of services. i.e. someone 'needs' day care or residential care. Under the Care Act it is no longer acceptable to define needs as an absence of service The Care Act articulates needs in terms of health and wellbeing outcomes that have to be achieved, and which focus on the assets or strengths of the person
