Market primary care services for the uninsured and underinsured in Lafayette and surrounding counties Extend dental services for Medicaid patients in the region Collaborate with existing providers in the region to coordinate specialty care Recruit additional providers to provide healthcare for the vulnerable Rural Community Health Center (FQHC) Provide access to group health insurance to small businesses in Lafayette County Improve wellness initiatives in worksites through WellnessWorks and 4EverFit websites, on-site coaching and screenings for members Increase access to local health care providers through incentive programs Rural Missouri Health Co-op (Insurance Plan) Development of Lafayette County Tele-Medicine/Distance Learning Network Expand coverage of tele-medicine by developing Western Missouri Rural Tele-Medicine/Distance Learning Network Development of co-op agreements with University of Missouri for Regional Extension Center activities (Meaningful Use Measures) Health Information Technology

Assist in the Development and funding of the Saline County Health Care Network Establish a Dental Clinic for Medicaid patients in cooperation with Saline County Health Department and AHEC Recruitment of additional hospitals and clinics in the regional area as members of HCC for tele-health/telemedicine purposes Network Expansion and Mentorship Research and Development of three health fairs for elders (screenings, medication management and speakers) Present data collected through the health fairs to area providers, enlisting their support to recruit specialty providers of services for a rapidly aging population Develop extended care pathways for chronic disease conditions Coordinated Care for Elders Ongoing membership recruitment for general membership and committees Partner with the 4 Life Center to develop local ownership of FQHC and to develop new, innovative programs for 4 Life Tenant collaboration Partnership with regional healthcare providers for specialty services, providing those services at a local level Collaboration Health Care Coalition of Lafayette County Initiatives

Rural Missouri Health Co-op First Steps….24 months Survey the service area for interest in insurance product Involve health care providers as part of an incentive plan Submit an RFP for a broker partnership Secure a broker with like-minded principals Obtain an insurance agent (producer) license Develop a marketing plan Develop a website (wellness focused) with assessment tools (WellnessWorks.HCCNetwork.org) Develop agreement with broker for membership structure and marketing fees Sign agreement with broker and begin marketing membership with insurance benefits

Coupled with wellness program for maximum return on premium reduction Incentive plan for local providers Wellness Coach Any member of HCC can purchase services Wellness Survey Establish goals and objectives of employee wellness Develop a wellness team, budget and timeline Internal promotion campaign Register participants Determine benchmarks Evaluate/monitor program Provide employer with health related statistics and other data of employees
