Use the folders on the left side of your screen to select the type of form or template you need. Click the name of the document to view the form in the Preview Window. Click the name of the document in the File Information box to download. Make sure you save it to a place you will be able to find it later. Using the template library

Open a word document Select the Margins In the header right click select “Edit” Using your Merge Tag reference sheet Add a Barcode With File Name Patient – First Name, Last Name Patient - Birth Date (mm/dd/yyyy ) Note: Barcode should be 20-24 font size the 3 of 9 Barcode Adding a barcode to a form

Creating a form Open a word document Select View Rules about templates Select the appropriate merge tags, copy and paste or type the merge tags Add the body of the letter Save as “RTF” and name the document

Uploading a Form Select the “Tools” Tab From the Navigation Tree select: Office Documents Manage Template From the Tools detail window: Select the Upload a template to the system

Uploading a form Browse and select form Select tab, enter document name and select the type of document Press “Upload” Verify the document title is correct and placed under the appropriate document “tab”

Test the form for accuracy Select the document "Tab” Select “T” button Select “Open Read Only” Verify document is working correctly Print the document Scan document to Filing Center

2 backup USB hard drives (pictured bottom left). 1 is connected to server via a USB Cable (pictured bottom right) and a power outlet via a UPS unit Swap out the drives everyday –same time –store the one not connected to the server offsite or in a locked fireproof safe.
