6 Triad Alzheimer’s Collaborative Plan of Action 1. Establish a mechanism for SAFE RETURN registrations throughout Cape. 2. Train volunteers to conduct registrations. 3. Establish a common referral line. 4. Launch a marketing campaign for Alzheimer’s awareness: Radio, Public Access TV, Newspaper 5. Establish County SALT Council subcommittee on SAFE RETURN to meet biannually.

9 Referral Process 1. Family calls BCSO referral line & leaves message. 2. Triad Director gets paged when message is left. 3. Triad Director gathers message info and calls P.O.C. in town where registrant lives. 4. Local P.O.C. calls family and makes arrangements for a registration visit.

10 Registration Visit 1. How did you hear about SAFE RETURN? 2. Review cover letter 3. Review info sheet & answer questions 4. Sign release form 5. Fill out registration form 6. Take photo (not mandatory) 7. Give resource phone numbers 8. Mail info to Alzheimer’s Services in Hyannis

12 TASKS 1. Each town needs to select a P.O.C. If P.O.C. is a volunteer, you need to recruit others to assist in home visits. Use the buddy system! 2. Each P.O.C. should take 10 registration packets with them. 3. P.O.C.s need to communicate with their police departments to determine if a local registry exists, and if there is a separate local registration form. 4. P.O.C.s should see if a camera is available from the police department or council on aging. If not, contact David Vieira and we can work something out.
