Front bumper lifting lugs were damaged The radiator was pushed back Cover over the radiator was bent Covers over the motor were bent Fan blades were bent 13

Describe the loss The truck was out of service for two days. The potential loss The employee could have injured himself or others or worse. The truck could have been down for a longer period of time. 14

While traveling down the ramp in second gear with a load of crushed waste, he hit bumps on the ramp and lost control, hitting the wall at # 23 Safety Bay. 15

Slow down on ramp. Go down ramp in first gear when loaded. Be proactive. Report any changing conditions of the ramp to your supervisor. Reviewed this incident with all crews. Be aware of your surroundings and changing conditions at all times. 17
