2 SIM status update Recent highlights ▪ Delaware awarded State Innovation Model grant ▪ $35 million Cooperative Agreement Award for Model Testing Assistance ▪ Project Period 2/01/2015 – 1/31/2019 ▪ RFIs for Practice Transformation and Care Coordination CMMI grant application ▪ Delaware Center for Health Innovation (DCHI) Board met December 10 ▪ Committees have begun meetings DCHI ▪ DHIN continues to work toward the Common Provider Scorecard Technical Advisory Group

3 Reminder: SIM grant Principles of funding Focus grants on start-up / one-time costs; find alternative sustainable funding for ongoing operational costs Build capability of local staff to operate program and eventually take over full responsibility Preserve SIM grant funds for uses with limited alternative options Use grants to seed matching investments from stakeholders, whether explicit or implicit Leverage grants from foundations wherever possible Leverage Medicaid to obtain a favorable federal match Establish that direct investments in delivery system should be funded by payers Leverage support (monetary and in-kind) from stakeholders SIM grant Delaware submitted a SIM Model Test grant application on July 17, 2014 ~$40M was requested and is part of a total investment of an estimated $139M over 4 years for the SIM program The remaining ~$100M will be a combination of in-kind donations of time, other state and federal resources, foundations grants, and stakeholders contributions, of which ~$60M will go towards care coordination fees Delaware Health Care Commission is the applicant for the grant and will follow applicable state and federal rules for uses of funds
