Transfusion Reaction Responsibilities STOP transfusion stat & disconnect Maintain IV site-disconnect from IV and flush with NS Recheck ID Notify blood bank/MD Monitor VS Treat sx per MD orders Save bag and tubing-send to blood bank Document per policy
