2 2009 Your Opinion Our Future SurveyClarify and Prioritize Root Cause Root Cause is an identified reason for the presence of a defect or problem. The most basic reason, which if eliminated, would prevent recurrence. Determining the root causes is a critical step to developing action plans focused on the right issues. The following slides provide an overview of the 5 Why’s technique for determining root cause of an issue.

3 2009 Your Opinion Our Future SurveyClarify and Prioritize The 5 Why’s Process 1.Describe the specific problem. Writing the issue helps you formalize the problem and describe it completely. It also helps a team focus on the same problem. 2.Ask “Why” the problem happens and write the answer down below the problem. 3.Evaluate the “Why” against the facts of the problem. 4.If the answer you just provided doesn't identify the root cause of the problem that you wrote down in step 1, ask “Why” again and write that answer down. 5.Loop back to step 4 until the team is in agreement that the problem's root cause is identified. Again, this may take fewer or more times than five Whys.

5 2009 Your Opinion Our Future SurveyClarify and Prioritize Based on survey results, determine areas of focus. Brainstorm with your staff, ideas for action plans to address the areas of focus. Utilize the 4 Block #1 tool to focus on the ideas meet the following criteria:  The idea impacts a large percentage of employees in your department.  Your department has direct control of implementing the idea.  Add the ideas to the 4 Block based on the above criteria. 4 Block #1 - Control & Employee Impact

7 2009 Your Opinion Our Future SurveyClarify and Prioritize Utilize the 4 Block #2 tool to transfer the ideas that includes high department control and has an impact on the largest number of employees. Then answer the following questions:  Is the idea easy or difficult to implement?  Is the idea high or low in cost? Based on the answers to the questions above, select the ideas for your action plan that has the following criteria:  High department control.  Impacts the most employees.  Is not difficult to implement.  Is not high cost. 4 Block #2 - Cost & Ease of Implementation
