Imagine the result Do I Have to Be Medically Evaluated? Yes. Driving a CMV places physical and mental demands on the driver that requires medical evaluation. ARCADIS manages the physical examination process through WorkCare (with a few exceptions not addressed in this training). Contact WorkCare to arrange a medical examination at a WorkCare facility near your work location. If you are already participating in medical monitoring under HAZWOPER, you will automatically be examined for DOT purposes when you take your HAZWOPER physical. You are required to renew the medical evaluation every 2 years. WorkCare 1-800-455-6155

Imagine the result Does That Include Drug and Alcohol Testing? For most employees – NO. Controlled substances and alcohol testing are separate from the medical evaluation CDL drivers are required to participate in controlled substance and alcohol testing More on those requirements later in the presentation “I will ask you about what medicines you are taking, including over-the-counter medicines.” – Dr. Askalot

Imagine the result The Medical Examination Report… Is a multi-page form filled out by you and the physician - The facility may require you to complete additional forms related to this report The medical facility will submit the report to WorkCare WorkCare will keep the report on file and manage information related to the medical examination

Imagine the result The Medical Examiner’s Certificate… Is proof you are physically and mentally qualified to operate a CMV You will be given the original copy of this certificate when you leave the medical facility Make several copies and keep in a safe place YOU are expected to carry this certificate (original or copy) when driving a CMV! DO NOT LOSE THIS CERTIFICATE!

Imagine the result What will the Doctor Evaluate? The Doctor will evaluate you in the following areas: ✓ General Appearance and Development ✓ Head and Eyes (vision) ✓ Ears (hearing) ✓ Throat ✓ Heart ✓ Blood Pressure ✓ Lungs ✓ Abdomen and Viscera ✓ Genital-Urinary and Rectal Examination ✓ Neurological ✓ Spine and Musculoskeletal Condition ✓ Extremities This examination can be done in conjunction with your HAZWOPER physical, if you participate in that medical monitoring program.

Imagine the result What Medical Conditions Might Keep Me from Operating a CMV? Generally, a driver is considered medically qualified if they do not have the following conditions that could interfere with safe operation of a CMV: No loss of arm, leg, hand or foot No impairment of a limb or digit No diabetes requiring insulin to control No heart problems No respiratory problems No blood pressure problems No medical history or clinical diagnosis of rheumatic, arthritic, orthopedic, muscular, neuromuscular, or vascular disease No clinical diagnosis of epilepsy or any other condition that might result in loss of consciousness No mental, nervous, organic, or functional disease or psychiatric disorder No vision (including color identification) or hearing problems No current substance abuse problems or diagnosis of alcoholism Remember, you can't have any of these conditions if it affects your ability to safely operate a CMV.

Imagine the result What About Prescription Medications? The physician may discuss this issue with you Per FMCSA, a Driver cannot use a controlled substance identified in 21 CFR 1308.11 Schedule I, an amphetamine, a narcotic, or any other habit forming drug unless the substance is prescribed by a licensed medical practitioner who: Is familiar with the driver’s medical history and assigned duties - and - Has advised the driver that the prescribed substance or drug will not adversely affect the driver’s ability to safely operate a CMV However, ARCADIS does not permit operation of any vehicle while under the influence of controlled substance or medication described above

Imagine the result I Have One of Those Conditions that Might Affect Safe Operation – Does that Mean I Can’t Operate a CMV? Not necessarily You can apply for a Skills Performance Evaluation (SPE) Certificate and, if approved, you may be permitted to operate a CMV Rules are complex and beyond the scope of this training Some conditions may not be subject to the SPE process Contact the ARCADIS DOT Compliance Manager or ARCADIS Medical Monitoring Administrator for more information The SPE Certificate, if granted by the FMCSA, must accompany your Medical Examiner’s Certification and be maintained in your Driver Qualification File (discussed later in presentation)
