4 4 The County’s Health Care Commission completes a 2006-10 community health assessment, implementation & evaluation plan. SCCHCC introduces Get Up & Go! Campaign at November Conference. Get Up & Go! Campaign & PHC Leadership Team prepares community action plan. Get Up & Go! launches 2008 Campaign with a 30- day challenge and Celebration event. PHC Team implements 14 Community Cooperative Agreements. County Health Department supports Get Up & Go! Community demonstration projects through federal prevention funds. 200620072008200920102011 St. Clair County’s Timeline towards a Shared Vision of Health Quality of Life Survey identifies need for “Community Connectedness.” 2 nd Annual Health Policy Summit –“Food for Life.” and 3 rd Annual Get Up & Go! Celebration Event. 1 st Annual Health Policy Summit to create a shared vision for health. St. Clair County recognized as a Pioneering Healthier Communities member (PHC).

5 Big Vision. Very Little Funding We have Available Non- Cash “Assets” to leverage. -People -Parks -Buildings -Businesses -Ideas Lots of people willing to donate a slice of their time. But how to you coordinate things?

6 Project Description and Goal To convene a community health improvement planning process that facilitates cross-agency collaboration and results in an improvement plan with measurable health objectives and strategies for action. To integrate strategy aligned management techniques and tools such as strategy maps, balanced scorecards and community compact agreements into our Community MAPP and CQI process. Baseline – A meeting effectiveness survey was conducted among members of the St. Clair County Health Care Commission in 2008. The results showed that while 90% of the HCC members were actively participating in a MAPP Action Team, team strategies were not communicated among the Commission members. The survey also revealed an inability to evaluate program effectiveness. It also showed an overall willingness to participate in more joint projects. 6
