Data Recognition Corporation2 Participation in LEAP Alternate Assessment, Level 1 (LAA 1) Students who participate in LAA 1 must meet the requirements defined in the LAA 1 Participation Criteria form, be in grades 3–11, have LAA 1 indicated on their IEP, and be progressing toward a Certificate of Achievement.

Data Recognition Corporation11 Double Check the Response Document If the response document is not completed correctly, the student’s scores will be invalid and a zero will be reported for school accountability. Check that “Participation Level” column has one of the three levels darkened in every box. (20 circles darkened) Check that the “Score” column has one circle darkened in every row. (28 darkened circles under the entire column “Score”) If any circle is darkened in the “No Performance” column, check the “Participation Level”. Only the circle for “Introductory Participation Level” may be darkened. Check “Participation Level” and “Score” sections. There should be 48 circles darkened in these sections, combined. Data Recognition Corporation14 Security of Materials Response Document If additional Response Documents are required, add the Security Number (on page 4) of each Response Document to the School Security Checklist.

Data Recognition Corporation15 Security Checklists Each district and school will receive a Security Checklist. This form includes a description of materials sent to the school and a list of security numbers assigned to those materials. Use this form to track Instruments and Response Documents during the assessment.

Data Recognition Corporation16 Student Roster Record the Precode ID Number for each student who does not receive a preprinted Response Document. Student rosters are provided at the school level.

Data Recognition Corporation17 Response Document Precode ID The precode ID is the number under the bar code in the student information box on page 1 of the Response Document. The precode ID begins with “P.”

Data Recognition Corporation18 Document Return Form Located on page 32 of the Test Coordinators Manual Must be completed by each School Test Coordinator DTC should keep the original form and STC should keep a copy at the school Do not return this form to DRC.

Data Recognition Corporation19 Packing Materials Instructions for packing test materials to return to DRC are on pages 11–14 and 23–25 in the Test Coordinators Manual. Make sure that the used, scorable Response Documents are placed in the white plastic envelope and include the envelopes in box 1 of the return shipment.

Data Recognition Corporation21 Contacting UPS LAA 1 materials will be returned via UPS. Affix a UPS return label to the top of each box in the shipment. Call UPS at 1-866-857-1501 at least 24 hours in advance to schedule a pickup. Be prepared to provide the tracking number for box 1 of materials. Let the UPS representative know the number of boxes and the time of day your boxes will be ready for pickup.

Data Recognition Corporation22 Materials Return DRC provides school envelopes for returning used Response Documents, green DRC address labels, and white UPS Return Service labels. Materials may be returned to DRC before March 27; however, DTCs must ensure that Test Administrators have until March 23 to assess students if that time is needed to complete the assessment. Last day for DTC to call UPS to schedule pickup is March 26. Last day for UPS to pick up LAA 1 materials for return to DRC is March 27.
