Homeroom Teachers Please Collect: O State Consent Form for Scholarships, Grants and College Admissions O The state now requires that parents and/or 18 year old students provide permission for schools to share student data with the Louisiana Office of Student Financial Assistance (LOSFA) and to postsecondary institutions, such as Louisiana colleges. This consent form is required for all students. Please review the enclosed form and sign in the area to consent(permission) or to decline consent(no permission). The form should be returned to school by January 7 th. Please consider that the school may not share information required for TOPs or college admission without the signed form on file.
