Create new care plan in Standards Overview of Activities / Outcomes / Activity Sets Create new Activity Create an Outcome Put them together as an activity set

Activity Sets Activity Set Activities 1 or more (Interventions) Outcomes By using an activity set you can break down care elements and apply a frequency to items that require one. Ultimately this allows for electronic sign off. On the next page I have broken down an example of a hearing deficit

Breaking it Down Hearing Deficit – Wears Hearing Aids Apply aids daily Frequency – Daily 0800 Check batteries daily Daily 0800 Store hearing aid in container in top drawer Daily – 2000 Resident able to hear and communicate adequately Review Frequency - Monthly

Type 1- Standard (RCS Style) Standard Style – 1 care plan per topic eg Personal Hygiene - show Sample Advantages One stop shop Familiar – similar to paper version Disadvantages Less personalised Decreases ability to link properly to assessments Example - Communication – Ellen Brown

Type 4 – Detailed Activity Sets Similar to the previous version but using activity sets rather than single activities Enables structured setting of Outcomes Structure interventions with different frequencies Example – Joan Smyth

Summary Smaller more detailed care plan items Make them specific Use the Assessment tool to design what is required on the care plan Care Plans can actually be shorter with only information specific to the resident needs eg Care of hearing aid…
