ARD C OMMITTEE M EETING Confirms, on the basis of all options discussed and considered, that residential placement is required. Addresses the items/questions on the application Student’s IEP must: Contain specific goals and objectives Address the services for which funding is requested in the application Ensures that a representative from the facility attends the meeting or participates by phone or conference call. 10/25/2015 13

R EINTEGRATION P LAN Plan is used to return the student to the LEA Include the provision of noneducational services For monitoring purposes include: criteria for the student’s return expected return date Progress on the goals and objectives for return to the LEA is benchmarked 10/25/2015 16
