16. How Will the Survey be Administered for Option 2 1.Complete the spreadsheet of OR Personnel and send it to us 2.We will send you a draft cover letter that you can modify for your institution. Send it back to us with your revisions. 3.We will send out an email with your cover letter to individuals at your hospital 4.Individuals will be able to opt out 5.We will send up to 3 reminders via email to those that didn’t respond or opt out
