4 Homework to Date Gather an implementation team Schedule a time and venue for a meeting to take place in 8-10 weeks Read background materials on the checklist and look at safesurgery2015.org website Create a list using the excel template provided to you of all of the OR staff, physicians, and techs. This template asks for names, titles, and email addresses Send us a picture of your checklist implementation team Designate if your hospital will be participating in the research portion of this project

15 Assessing the Culture in Your OR This survey is available to everybody If you don’t participate in the research we highly encourage you to administer it yourself Process for administering the survey for Option 2 participants will be automated

16 How Will the Survey be Administered for Option 2 1.Complete the spreadsheet of OR Personnel and send it to us 2.We will send you a draft cover letter that you can modify for your institution. Send it back to us with your revisions. 3.We will send out an email with your cover letter to individuals at your hospital 4.Individuals will be able to opt out 5.We will send up to 3 reminders via email to those that didn’t respond or opt out

18 What Happens When there is a Low Response Rate? We will send you a list of people that didn’t respond We ask that you follow-up with them individually and to complete the survey You send us the completed surveys

19 Strategies for Getting Physicians to Complete the Survey Stand outside of the OR with the survey and a pen/pencil/laptop Sit in the MD Lounge with food and ask them to complete the survey

20 What to Take Home Understanding where you are right now will impact the way you implement You may be surprised by what you find when see the survey results Survey can give us valuable information to measure your progress with putting the checklist into place

21 Homework If you haven’t already, designate if your hospital will be participating in the research portion of this study –Send an email to: Lizzie Edmondson at: safesurgery2015@hsph.harvard.edu If you are in Option 1 start administering the survey If you are in Option 2 send us your completed spreadsheet that includes (anesthesia providers, nurses, surgeons, and techs).
