In South Hampton Roads: YOU Approximately 160,000 people were without healthcare in 2011 & 2012, but YOU raised over $29,000 towards local CFC health charities in 2012! $52/year supports 12 elementary school students with trained volunteer tutors to help with reading & math. Did you know that 5.3% of public school kindergarteners are retained & the percent of students graduating from South Hampton Roads high schools is 62.5%. YOU But YOU raised well over $27,000 in 2012, in support of local youth charities! In Virginia: YOUR Losing a job leaves people with worries about their financial stability & future, but YOUR support of over $1M helped many unemployed Virginians. In America: There’s a significant rise in the suicide rate of our returning Veterans & in many cases the Veterans’ inability to care YOU for his/her family is significant, but YOU have shown great support with contributions of over $295,000 towards charities serving Vets & mental health charities. It is estimated that up to 25% of the total amount of carbon dioxide going into the atmosphere every year is YOUR released from trees being cut & burned. But YOUR 2012 contributions over $145,000 supported education on ways to help prevent increased air pollution in communities. Around the world: Every 20 minutes, the world loses one or more entire species of animal or plant life – at least 27,000 species per year, but YOU YOU have raised over $221,000 in 2012 in support of animal & wildlife charities. A portion of people, of the 884 million that lack access to disease free water, now have fresh water YOUR thanks to YOUR CFC contributions. 6

Visit www.cfcshr.orgwww.cfcshr.org Campaign Account Report Review your command/agency’s campaign history— Campaign Account Report Involve & seek the support and commitment of your Leadership Key Workers Select co-workers (Key Workers) to assist you Host a kick-off activity; include CFCSHR speaker presentation, fundraisers & internal incentives (Please be sure first to check with your JAG/Ethics Officer regarding fundraising activities.) Run a Leadership Giving Campaign which involves Senior Leadership & donors who give at a leadership level of $240 annually 8

Submit ALL carbon CFC Copies and ALL monies (cash/check) inside Campaign Report Envelope to the CFCSHR office. Submit ALL carbon PAYROLL/PSD Copies and ALL NEXUS Payroll copies (signed by donor) inside Payroll Envelope to your Command Pass Coordinator (CPC) or Federal Agency’s Payroll Office. 12 Complete a separate Payroll Envelope for Military & Civilian pledges. DO NOT submit both type in same envelope as payroll offices will not accept.

13 Complete form with your name, Agency/ Command name and Work address & Phone # Enter your ann’l total pledge amount or feel free to select an amount listed, for your convenience. Do you want your charity(ies) to contact & thank you? If yes, enter your personal contact information. DO NOT enter your work email. Is your gift at a leadership level of $240 or more? If so & you wish to receive a donor leadership recognition gift, select appropriate gift. Enter your SSN/Empl ID ONLY IF pledging via Payroll Deduction If your pledge IS NOT payroll deduction, select Cash/Check and enter amount. Enter up to 5 Charities you wish to give to. Transfer an apportioned amount of your total gift to each charity. Otherwise, your gift will be undesignated. Be sure to Sign & Date your pledge card ONLY IF you are pledging via Payroll Deduction. Eagle Award ($1500+) Donors who contribute at a minimum leadership giving level of $1,500 annually to the CFC South Hampton Roads campaign are eligible to receive the Eagle recognition gift. This recognition gift is a personalized CFCSHR Padfolio Writing Pad. The Beacon Award ($500-$1499) Donors who contribute at a minimum leadership giving level of $500 annually to the CFC South Hampton Roads campaign are eligible to receive the Beacon recognition gift. This recognition gift is an original CFCSHR personalized Tervis Tumbler. The Flag Award ($240-$499) Donors who contribute at a minimum leadership giving level of $240 annually to the CFC South Hampton Roads campaign are eligible to receive the Flag recognition gift. 30 The South Hampton Roads CFC Campaign Awards Program consists of recognition categories with specific criteria, designed to celebrate and recognize the creative efforts and campaign achievements of the Federal Agencies/Military Commands, Federal Civilian & Postal employees, CFC Coordinators and Volunteers. GoalBuster Awards – Agencies/Commands that exceed dollar goal Participation Awards – Agencies/Commands that have achieved 50% participation or more Most Innovative CFC Campaign Awards – Agencies/Commands that have demonstrated the most innovative & creative CFCSHR campaigns. CFCSHR Leadership Award – A recognition is given to an individual CFC Coordinator who best exhibits a high spirit within the CFC campaign in completing their job enthusiastically and effectively by organizing, planning and implementing a successful workplace campaign within their Federal Agency/Command. CFCSHR Hero Award – A recognition is given to an individual CFC Volunteer who best exhibits the spirit of CFC in volunteering their time to a worthy cause and or charity, including a charity Board participation, etc. Command Area Achievement Award (left) – Presented to the Command Area in recognition of the most outstanding CFC campaign in South Hampton Roads.
