Eligibility Requirements ALL applicants must be: 1.A public or nonprofit private entity, including tribal, faith-based & community-based organizations; and 2.All applicants must propose to serve the same service area and/or populations identified in Appendix D of the funding opportunity. This includes: –Grantees whose project period ends on or after Oct. 31, 2010 and before Oct. 1, 2011. –A new organization that can serve the entire service area and/or population identified in Appendix D of the funding opportunity. New organizations include an organization representing a consortium of health centers who through their partnership can serve the entire service area and/or population.

Eligibility Requirements cont’d All competing organizations must: Provide services to the entire announced service area; Provide services to the entire population currently being served (applicants may not propose to serve only a segment of the existing population being served); Provide the same or comparable comprehensive primary health care services presently being provided to the population; Utilize Federal funding to serve patients currently served by the existing grantee; and Request an equal or lesser amount of Federal funding as currently received by the existing grantee, including funding for special populations (migrant, homeless, and public housing programs).

8 review criteria—NEW *Criterion 3-Collaboration Describe formal and informal collaboration and coordination of services with other health care providers (e.g., existing section 330 grantees, FQHC Look-alikes, rural health clinics, critical access hospitals, etc.) Provide letters of support from current FQHCs, FQHC Look-alikes, rural health clinics, and critical access hospitals OR provide justification as to why such letters cannot be obtained Program Narrative

Program Specific Forms & Program Specific Information All forms except for Form 3 (Income Analysis Form) are now structured

Clinical Performance Measures 8 required performance measures (6 standardized measures plus one oral health and one behavioral health measure): –Prenatal care –Childhood immunizations (for 2 year olds) –Pap tests –Hemoglobin A1C test results for adults with diabetes –Low birth weight babies –Blood pressure readings for adults with hypertension –Behavioral health –Oral health

Clinical Performance Measures cont’d Option to include additional health care- related measures of their choice Any additional narrative regarding the Clinical Performance Measures should be included in the Evaluative Measures section of the Program Narrative, as appropriate

Financial Performance Measures 5 required financial performance measures: –Total cost per patient –Medical cost per medical visit –Change in net assets to expense ratio –Working capital to monthly expense ratio (current financial condition) –Long term debt to equity ratio (long term financial conditions) (Exception: public and/or tribal entities are only required to complete the 2 non-audit related measures)

Financial Performance Measures cont’d Option to include additional financial- related measures of their choice Any additional narrative regarding the Financial Performance Measures should be included in the Evaluative Measures section of the Program Narrative, as appropriate

Electronic Health Records (EHR) Required—questions 1 and 4 are mandatory for all applicants Information submitted provides HRSA with data—not used as an awarding factor It is a structured form

Differences Between SAC 2010 and SAC 2011 Funding Opportunities Review Criteria point allocations have been changed New Collaboration criterion (Criterion 3) added as part of the Program Narrative Health Care and Business Plans are now referred to as the Clinical and Financial Performance Measures

Additional Information Existing grantees must submit a 5-year Budget Justification; new applicants must submit a 2-year Budget Justification 150 page limit
