Stages of OT Process: Referral
Point at which initial request for services for particular client at a change in the degree and direction of services is made Self – referral Referred by a physician, another professional, or another agency

Stages of OT Process: Screening
gather preliminary information about the client

Screening process may involve: review of the client’s records
use of screening test interview with the client and caregiver observation of the client discussion of the client’s referral source determine whether further evaluation and OT intervention is warranted

Assessment – sum of the evaluation results which provide a composite picture of the client’s level of function Process where: data gathered about the client problems are identified hypothesis is formulated decisions are made for treatment intervention

client is discharged from OT when he has reached the goals delineated in the treatment plan or when it is determined that he or she has realized the maximum benefits of OT services

Discharge Plan and Summary
Discharge plan – conforms to the personal goals and addresses the resources available in the community Discharge summary – summary of the status of the client’s functional level at the time of discharge, it may contain the plans for discharge, equipment and services recommended after discharge
