Creating a New Form Click More to find Google Forms.

In the top left corner, click on Untitled Form and give your document a name.

In Form Settings, uncheck Require PPS login to view this form.*
Click OK. Add a Form Description in the text box provided. It can be left blank. * Do this so anyone can access the form without being logged into a Google account.

Creating Questions in a Form Text Questions
Click on the pencil icon to edit your question. Type your question in the Question Title box. The Help Text box can be used to give additional directions for the question. From the Question Type drop down menu, choose Text.

Check the box next to Required Question if you want to make sure the user completes the question. Click Done. A red asterisk will appear on required questions. Use the Duplicate icon to copy the question you just created to modify the question without having to redo the settings. Change First Name to Last Name. Click Done.

Paragraph Questions Click on Add Item to begin a new question.
Write your next question, this time add directions in the Help Text box. From the Question Type drop down menu, select Paragraph Text. Click Done.

Multiple Choice Questions
Click on Add Item to begin a new question. From the Question Type drop down menu, select Multiple Choice. Add Other as a choice so the student can give a response if the one they want is not listed. Make this a required question. Click Done.

Checklist Questions Click on Add Item to begin a new question.
From the Question Type drop down menu, select Checklist. Add Other as a choice so the student can give a response if the one they want is not listed. Make this a required question. Click Done.

Choose from a List Questions
Click on Add Item to begin a new question. From the Question Type drop down menu, select Choose from a List. Make this a required question. Click Done.

Scaled Questions Click on Add Item to begin a new question.
From the Question Type drop down menu, select Scale. Label the low and high end of your scale. Make this a required question. Click Done.

Grid Questions Click on Add Item to begin a new question.
From the Question Type drop down menu, select Grid. Make this a required question. Click Done.

Date Questions Click on Add Item to begin a new question.
From the Question Type drop down menu, select Date. Make this a required question. Click Done.

Time Questions Click on Add Item to begin a new question.
From the Question Type drop down menu, select Time. Make this a required question. Click Done.

Adding an Image to a Form
Click on the Add Item drop down menu. Click on image. Add an image By URL, Click Select. Resize your image by using the blue squares. Choose the alignment of your picture. Click Done.

Adding a Video to a Form Click on the Add Item drop down menu.
Click on Video. Do a Video Search. Click Select. Resize your video by using the blue squares. Choose the alignment of your video. Click Done.

Adding a Video Response Question
Click on Add Item to begin a new question. Write your next question, this time add directions in the Help Text box. From the Question Type drop down menu, select Paragraph Text. Click Done.

Installing Add-ons to a Form
Click on the Add-ons menu, then Get add-ons... Find the add-on you want. Mouse-over the add-on and click the free button to install. Clicking on the add-on will open a window with more details. A Request for Permission window will pop up. Authorize the add-on by clicking on Accept.

Confirmation Page The Confirmation Page is what appears after the user has submitted their form. Type a response

Adding a Theme Click on Theme... to add a header and style to your form.

Viewing Your Form Click on View live form to see what your form will look like to the user. When you are viewing the live form, copy that web address to send it out to whomever you want to fill out the form.

Viewing the Responses to Your Form
Click on View responses to see the answers from your form. Click Accepting responses to turn off your form.

Adjustments you can make to your Response Spreadsheet
Click on the blank cell in the top left of your spreadsheet to highlight the entire page. Under Format select Align, select Center and Top. Under Format select Wrap Text.

To hide a column, click on the upside down triangle in Row 1.
To view the column again, click on the triangle pointing in the direction of the missing column.

Your Challenge Create your own form: Name the form. Change the theme.
Include at least three different types of questions. Share the form with two people and ask them to fill it out. Share the responses with the instructor, a partner or whole group.
