4 The IS Report should include ALL data All relative data, whether or not funded directly by CSBG, should be reported.

5 Section A is a financial statement asking about the funds available for WV’s 2012 CSBG programs and how they were planned to be and actually were expended.

7 These sections ask for information about how WV’s CSBG programs are operated at local and state levels. Many of the questions ask for a yes or no response, or a check mark if it is a multiple-choice question.

15  This section asks for well written examples of the following: 1. Strategic Thinking for Long-Term Solutions 2. Delivering High-Quality, Accessible, and Well- managed Services  Top State Management Accomplishment  Top Agency Management Accomplishment 3. Mobilizing Resources to Support Innovative Solutions 4. Providing Positive Results for Vulnerable Populations.  One Youth-Focused Initiative  One Senior-Focused Initiative

16  Section E identifies only CSBG expenditures by service and demographic category.  Section F seeks to capture information more broadly describing all funds that support the entities providing CSBG services.

17 This section asks for CSBG expenditures in nine program categories. 1. Employment 2. Education 3. Income Management 4. Housing 5. Emergency Services 6. Nutrition 7. Linkages 8. Self Sufficiency 9. Health

18  The tenth category, “Other,” allows you to include programs that cannot be included under one of the nine service categories.

21 This section shows the programs or initiatives other than CSBG, administered by each CAA. It asks for an accounting of the CSBG resources in the local CAAs that fill out this section so that the mix of CSBG and other resources can be described to policymakers.

23 This section asks for demographic information on all those utilizing and benefiting from local CAA services. DBA FACS Pro Auto-Fills this information into Section G of the IS Report if the information was completed during Intake.

24  This portion of the survey gathers information on the 12 national performance indicators of community action.  The NPIs reported in this section were created collaboratively within the CSBG Network to enable the more than 1,000 diverse CAAs in 52 States and territories to present a more uniform and coherent national picture of their work and accomplishments.

25  The NPIs are about Community Action, not just those activities funded through CSBG. Outcomes should be counted and reported from all CA programs and activities.  Agencies should report outcomes only for those NPI categories in which the CAA has supporting programs or activities. It is not expected that all CAAs will produce outcomes for all of the indicators.
