ADAZI LSA AT/FP FOLLOW UP VISIT 6 & 7 JUNE

ADAZI LSA AT/FP ISSUES NOTED ON 2 JUNE: BOXES AND DEBRI ON ROAD TO LSA – POTENTIAL IED PLACEMENT MULTIPLE HOLES & BREACH POINTS IN ALL SIDES OF PERIMETER TRASH PILE SMOLDERING APPROX 65’ FROM FUELLER – PILE CONTAINS CANISTERS AND OTHER DEBRI FUEL TRUCK FOUND UNATTENDED OUTSIDE THE WIRE MOTOR POOL OUTSIDE THE WIRE, WITH ONLY 3 SIDES C WIRE – OPEN TO LOCALS NO GUARDS POSTED AT MOTOR POOL RUN-DOWN RESIDENTIAL AREA IN THE VICINITY OF THE LSA & MOTOR POOL DERELICT/ABANDONED BUILDINGS IN THE AREA, OUTSIDE THE PERIMETER LSA IS OUTSIDE OF MAIN INSTALLATION AND EASY TO APPROACH FROM ALL SIDES

FOLLOW UP: BOXES AND DEBRI ON ROAD TO LSA – POTENTIAL IED PLACEMENT ________________________________________ Boxes were still there. The site ATO was informed of the need for HN to remove the boxes.

FOLLOW UP: MULTIPLE HOLES & BREACH POINTS IN ALL SIDES OF PERIMETER ______________________________________________________________________ C wire was unavailable. It will arrive around 15 June (too late). Engineer tape was placed across holes & breach points as a deterrent. HN stepped up patrols around the LSA. HN stated they would fill the holes and breach points. U.S. personnel began daily perimeter inspections. Lights were emplaced around the perimeter. Interior perimeter was cordoned off with engineer tape as a deterrent.

FOLLOW UP: TRASH PILE SMOLDERING APPROX 65’ FROM FUELLER – PILE CONTAINED CANISTERS AND OTHER DEBRI ________________________________________ Trash pile was removed by HN, and the area was cleaned up.

FOLLOW UP: FUEL TRUCK FOUND UNATTENDED OUTSIDE THE WIRE ______________________________________________ Fueller was moved away from the trash pile area on 2 June. On 6 June, it was again found unattended, and this time, ungrounded. This was brought to the attention of on-site Commander. On 7 June, the fueller was found attended and grounded. Note: now that the trash pile has been removed, the fueller will be moved back to original location.

FOLLOW UP: MOTOR POOL OUTSIDE THE WIRE, WITH ONLY 3 SIDES C WIRE – OPEN TO LOCALS NO GUARDS POSTED AT MOTOR POOL ________________________________________________________________________________ Motor Pool found in the same condition on 6 June. None of the vehicles were locked. The on-site Commander had locks issued by 7 June. C wire not yet available

FOLLOW UP: RUN-DOWN RESIDENTIAL AREA IN THE VICINITY OF THE LSA & MOTOR POOL DERELICT/ABANDONED BUILDINGS IN THE AREA, OUTSIDE THE PERIMETER LSA IS OUTSIDE OF MAIN INSTALLATION AND EASY TO APPROACH FROM ALL SIDES ________________________________________________________ HN increased patrols in the LSA & Motor Pool areas. On-site ATO placed derelict/abandoned buildings off- limits. On-site ATO developed an in-processing brief to inform Soldiers of AT/FP issues at the site.

FOLLOW UP: INCIDENT UNKNOWN PERSON(S) AT AN UNKNOWN TIME ACCESSED THE INTERIOR OF THE LSA AND SPRAY PAINTED GRAFFITI ON ONE OF THE TENTS. DISCOVERED DURING WALK AROUND THE PERIMETER ON 2 JUNE. ________________________________________ It was determined the graffiti on the tent was old. The tent did not belong to the U.S. This is no longer considered to be an incident.

FOLLOW UP: MAIN GATE: HAS ONLY DROP BAR BARRIER POV’S PARKED IMMEDIATELY OUTSIDE THE GATE NO SERPENTINE TO SLOW TRAFFIC NO SEPARATE VEHICLE SEARCH LANE ________________________________________ The gate remains the same, however, parking in the lot by the gate has been limited.

FOLLOW UP: ORDINANCE USED AS DECORATION THROUGHOUT THE INSTALLATION ________________________________________ There is an EOD school on the base. On 7 June, they were conducting training with K-9 and robots. The ordinance was inert, and used for training purposes.

FOLLOW UP DIRECTION TO ON-SITE ATO: Immediately request HN remove the boxes near the motor pool. Follow up on HN fixes to the perimeter holes. Request HN post “KEEP OUT” signs around the LSA and Motor Pool in local language. Ensure Fueller is grounded & attended. Ensure Motor Pool is guarded & vehicles locked. *Provide a daily report to AT/FP at Tapa by 0930hrs.
