2 SSP & EHC Assessment requests SEND Support Plan: assess/plan/do/review New version available – reviewing is key, choice of interventions, showing your learning about the child’s sen along the way and how you have used advice and measured its impact One SSP for all 3 cycles, linear journey Importance of children’s views being sought, the child being involved in their reviewing

3 SSP & EHC Assessment requests Costs – be real, be honest, don’t need to show high costs to convince. Costs – under review, met with Heads yesterday, looking at ideas for realistic, equitable response to this Costs – projected costs will be requested at assessment stage Section F – influenced by professional recommendations and projected costs, but must be evidence of need for the provision

6 Conversion Review Meeting Social Care Pathways: First Response Team First Response Team (FRT) consists of six social workers, one deputy team manager and team manager. All contacts and referrals will be screened by the social workers within FRT to decide whether the contact is a referral and pass through to one of the Child Assessment Teams (CAT) on duty or sign post to an appropriate agency. SaLT – traded should reflect what is written in the statement Paediatric reports – can request update Hospitals – more difficult, reliant on family providing information (indicate to SMMO if you feel there is health information that you are unable to provide)

8 Conversion Review Meetings Transitions Focusing on transition from child to adult services Start looking at this early – health and social care need to make plans well before 18 Shona MacGregor (DPEP) – 14-25yrs focus on processes and pathways Linking with colleges and looking at employment and training opportunities for young people with SEND
