17 17 Reminder: the Reassessment The Reassessment Cycle requires that an OCAN be completed for each assessed consumer every 6 months Reassessment is an opportunity to: –Check if the support provided is benefiting the consumer –Reinforce the partnership between the consumer and staff –Review/celebrate the progress and revisit next steps
