3 You Should Be Proud!  Children are seen by the same doctor each time they visit your practice, which helps build trust.  For medically complex children, you work to update the care plan each visit and share a copy with the family.  Parents and caretakers are supported in being more involved in care decisions at your practice.  You make sure children are up to date on immunizations and provide guidance specific to their child’s developmental stage.  You are establishing relationships with resources in your communities in order to give your patients referrals to needed medical care and support services.

4 More accomplishments!  Offices are working cohesively as systems to meet patients’ needs. Internal communication is improved, as well as communication with families and with other providers in the community. The whole office is prepared before a patient comes in to meet his or her specific needs.  Prevention and chronic care management are addressed.  Care coordination helps patients get needed services.  More toddlers are being screened at 24 months.  Patients with asthma are more likely to be assessed for their asthma being in control and to have an asthma action plan reviewed and shared with the family.
