Behavioral Health Consultation Services - Pediatric a program to Support Behavioral Healthcare Practice in Pediatric Primary Care ---------- SmartCare BHCS is a behavioral health integration partnership program of Vista Hill. Funding provided by San Diego County HHSA Behavioral Health Services – MHSA.

Any Pediatric Primary Care Provider can… Consult with our Child Psychiatrists – Call our Clinical Triage phone line: (858) 880-6405 – Email us at bhcs-provider@vistahill.org.bhcs-provider@vistahill.org – Discuss clinical concerns about any of your patients – Review diagnosis, treatment, medication, ancillary care needs Use our screening tools, referral forms, parent information materials in your office. Refer Parents to our clinical team for care planning support – We will review parental concerns, collect basic behavioral health data, and then provide guidance and recommendations for needed care. A brief report will be sent to your office. – BHCS does not provide treatment ---but we can help with planning and referral for ongoing treatment. Refer Parents to our Parent Education Programs – Weekly community meetings where parents can learn more about behavioral health concerns and how to navigate the system.

When you call: Our Nurse Practitioner will take your call directly. She and the child psychiatrist will address your concerns promptly. We FAX a brief consultation note back to your office. Your clinic nurse or assistant can initiate your call for your convenience.

How long will it take? Not long. We aim to be there when you call and will provide a definitive response within minutes. You may have your staff initiate the call to provide background information and have us on the line when you pick up. You can also schedule a consultation session at your convenience to review several cases or discuss any relevant clinical concern. Emails and econsults can be sent at your convenience after hours– with 24 hr turnaround response time.

Pediatric Patients: between 0 and 22 years old Screening, Assessment, Treatment Concerns – General Behavioral Health Conditions ADHD, Behavior Disorders, Depression, Anxiety, Sleep Disorder, etc. Family Problems, Adjustment Issues, School Difficulties, etc. – Severe illnesses and Disorders Autism, Bipolar & Mood Disorders, Psychotic Disorders – Substance Abuse Medication management support – Appropriate Medications to Prescribe – Dosing & Titration Strategies – Side effects & Contraindications Crisis Assessment and Management Strategies – Suicide Risk, Danger to Others, Risk Management – Community Referrals & Other Resources

We will do our very best to help.

Will I receive a written consultation? Yes. We will fax a concise report to your office. “ Dear Dr. Jones: Thanks for your call. As discussed, it appears that your patient is experiencing an exacerbation of ………. Based on the information available, it seems appropriate to titrate the patient’s medication in the following manner. 1) ……………… 2) ……………… If needed, please contact us multiple times to discuss your patient’s needs. Will BHCS- Pediatrics prescribe medication? NO. – BHCS-Peds offers only consultative recommendations regarding medication management. Prescriptions and the oversight of ongoing medication are the responsibility of the PCP. Follow-up consults are available to support Primary Care Providers with on-going care. When needed, we can help with referral to specialty mental health providers.

Screening for Behavioral Health Problems Don’t ask, Don’t Tell is still alive! – Patients and parents do not always ask for help. Simple screening tools can lead the way. – Our one page form can be used in your office. Diagnosis-specific tools can help in monitoring the course of an established patient. Other resources are available for your office.

BHCS – Pediatric Services for Parents Parents can call our clinical team – Refer parents to call our Parent Helpline (858)956-5901. Use our Consult Referral Script. – We will explore the parent’s concerns and collect basic behavioral health data. – We’ll provide guidance regarding needed care and send a brief report back to your office. Refer Parents to our Parent Education Programs – We provide weekly community meetings for parents to learn more about behavioral health concerns and how to navigate the system – Meetings held in four sites throughout the County.

BHCS Provides FOR PROVIDERS – Child Psychiatric consultation and support. – Office tools to advance behavioral health care capacity. – Summary of relevant clinical information obtained from parents. FOR PARENTS – Telephone consultation and education for parents.................. BHCS does not provide: – Direct treatment services for children or families – Medication prescription

What Does This Cost? The BHCS – Pediatric service is FREE. – There are no charges to the provider. – There are no charges to patients. – There are no insurance forms to complete. Funding for SmartCare BHCS – Pediatric is provided by San Diego County HHSA Behavioral Health Services – Mental Health Services Act (MHSA).

What if the question arises after the hours of 8:30-4:30? If you have a consult question after hours, you may contact us to leave a message. When feasible, we will respond----most after hours contacts will be responded to first thing the next business day. For urgent matters after hours, contact the San Diego County Access & Crisis Line: 1-888-724-7240, available 24/7
