(1) First Aid Sore Left Shoulder Strained shoulder joint Employee was bolting back when the drill he was using jammed (probably hit a rock bolt) The stoper gave a jerk and employee felt a pull on left shoulder. (Suspect bolt was hit due to angle of adjacent rockbolt. Worker was using a 6’ steel at time. Left arm became over extended as rotation took off when steel jammed.

(2) First Aid - Pain in left shoulder Worker was installing a (10’), 4” Heavy Wall Pipe in chain then step over into scoop bucket when worker slipped, the pipe came down out of chain. Worker was still hanging on to pipe and stretched arm out hurting his left shoulder.

(3) First aid - Sore Right Ankle Worker was installing 5’ rebar and screen straps on the wall where raise had bumped. Crew had lots of loose to scale and pieces landed on work staging while scaling. Existing screen was holding some loose and worker was removing loose from screen when he slipped on a piece of muck on staging and legs did the splits. This caused the worker to twist right ankle.
