All Rights Reserved MoM Communities - Introduction What are MoM Communities? It is suggested that a MoM Community should comprise a cohesive group of primary and secondary care organisations who individually provide care to the same group of patients. The MoM Community will most probably, but not necessarily be identical with the Local Health Community. The Community is able to customise and annotate their ‘variant’ of MoM to reflect their specific local processes and practices thereby enhancing the value and usability of the pathways to their healthcare providers. What are the Benefits to users of joining a Community? Functionality of MoM Release 3 –Allows sharing of local information such as clinic times and local support group contact details available across the Community –Ability for individuals to submit comments or suggestions to enhance their local variant of MoM –Ability to add, delete and customise all content including nodes, pathways and departments to best reflect the local services and practices of the community –Option to accept or reject changes to the NHS Standard Pathways

All Rights Reserved What should be considered when setting up a MoM Community? To setup and manage a successful MoM Community a number of points should be taken into consideration: 1.Who are the stakeholders? 2.What is the overall objective of a Community review board? 3.What is a LHC – is this a practical boundary for a MoM Community? 4.What process will be used to manage changes to the Variant? 5.How is the Community going to be implemented across a number of varied organisations and in co-ordination with other deployments? 6.How is the Variant going to be maintained on an ongoing basis With respect to deciding which pathways to prioritise for local customisation, the following processes may be helpful: 1.Known problems for particular specialties (e.g. long waiting time for Neurology out-patient clinics) 2.Information from existing audits that have identified areas for improvement 3.Acknowledged national/international expertise 4.Local initiatives to tackle diseases

The community should be structured in such a way as to enable effective management of their MoM variant. Ideally the community should: –Take ownership of its MoM variant –Have specific people to fulfil essential business functions required to manage administrative note changes and their MoM variant: –Have a documented process for making controlled changes to the MoM variant: Resulting in an audit trail of signatories Including review of changes by a clinician with specialist knowledge of relevant pathways –Have a strategy for engaging clinicians to encourage their adoption of MoM. This could include: Direct engagement with organisations Demonstrations at existing meetings Additional Business Functions in R3 Map Display Users Feedback Reviewer Admin Editor Map Manager Users Editors Approvers/Reviewers Publishers
