3 Health Care Organizations and Long Term Care STAKEHOLDER RESPONSIBILITIES Incentives –Align incentives around value propositions to define priorities –Mobilize financial and workforce resources –Engage all types of healthcare entities – primary/specialty care, community hospitals, long term care, pharma, dental etc. –Comply with mandated interoperability standards
