MPO antibodies in Churg-Strauss Syndrome 논문에 따르면, CSS 환자의 약 38% 가 IFA 검사에서 양성. 이 양성환자의 약 ¾ 이 MPO antibodies 에 의함 (30%) (Sinico RA et al, 2005, Arthritis Rheum 52:2926-35) 자사 평가에 의하면 60 명의 CSS 환자 혈청 중 23 명에서 ANCA pattern 이 나타남 (38.3%). 하지만 단 10 명만이 IFA 검사결과 전형적인 pANCA pattern 을 보임 (16.7%). 11 명의 혈청에서 MPO antibodies 가 검출됨 (18%). EliA MPO S 보다 민감도가 좋은 타사 제품은 없었기 때문에, 낮은 민감도는 EliA 때문이 아니라 sample panel 때문에 발생. 평가에 사용된 Churg-Strauss 검체에서 anti-MPO antibodies 의 prevalence 가 낮아 2 번의 assay 결과를 가지고 계산 (with and without Churg-Strauss sera).

The classical MPO- associated pANCA pattern was found in 35.5 % of AAV and 2.7 % of controls. *x-ANCA: in most cases positive in ethanol-fixed granulocytes but negative in formalin-fixation. **Measurements are based on the MPOs serum panel (see slide 8) plus 30 additional sera from patients with Wegener’s granulomatosis Almost 30% of non-AAV are positive in IFA. pANCA is less sensitive and less specific than EliA MPO S. The clinical usefulness of IFA as screening test is questionable.

20. EliA MPO S - 임상적 검사성능, 요약 EliA MPO S 검사는 기존의 EliA MPO 보다 민감도 향상. EliA MPO S 검사는 타사 검사법보다 검사성능 우월. EliA MPO S 는 Wieslab 의 Capture-MPO-ANCA ( capture assay) 보다 약간 더 우수하고, 이 두 가지 검사만이 IU/ml (standardized to the CDC standard) 을 사용할 수 있기 때문에 최고의 검사법으로 인정. IFA 는 AAV (ANCA associated vasculitis) 에서 민감도가 높지만 특이도는 아주 낮음. Disease control 의 ¼ 이상에서 ANCA 양성 (granulocytes) 의 결과를 보임. pANCA 는 EliA MPO S 에 비해 민감도와 특이도가 모두 낮음. 따라서, IFA 의 임상적 유용성은 의문의 여지가 있음.

21. EliA GBM for detection of anti-GBM in ANCA- positive samples ANCA 양성 환자는 GBM antibodies 를 반드시 확인해야 한다 ! ANCA 양성 검체의 5-14% 정도는 anti-GBM 도 양성. Anti-GBM 음성인 vasculitis 환자보다 두 검사에서 모두 양성인 환자가 질환에 대한 예후가 더 심각. (Levy et al, 2004, see PoM 01/05) 빠르게 악화하는 glomerulonephritis 로 의심되는 환자 중 약 2% 가 ANCA 와 anti-GBM 모두 양성. 예후는 ANCA 만 양성인 경우가 anti-GBM 만 양성인 경우보다 좋다. (Jayne DRW et al, Kidney Int 1990; 37:965-70)
