2. This accident was caused by a drunk driver in Conway, SC. She knows she rolled at least two times, but can’t remember everything because she was too intoxicated. She was wearing her seatbelt. This accident was caused by the driver text messaging. The employee lives in Farmington, NM. The accident occurred in Colorado. None of the four employees inside of the truck were buckled up.

4. Additional Information While teaching the Incident & Injury Free Orientation on May 29 th this accident was shared. The driver later e- mailed me the pictures. None of the employees in the truck are over 30. All of the employees proclaimed that they never wear seatbelts. No one was injured as a result of this incident. They drove the truck back to Farmington after it had rolled. The front passenger had to keep his head out of the window the entire trip because the roof of the truck was so compacted he could not sit up right in his seat.
